細(xì)胞型朊蛋白(PrP~C)在直腸癌中的表達(dá)及臨床意義

【摘要】:目的:檢測直腸癌組織、直腸腺瘤組織及相應(yīng)癌旁正常直腸組織中細(xì)胞型朊蛋白(PrPC)的表達(dá)含量,分析PrPC在直腸癌中表達(dá)水平與腫瘤分化、臨床分期及淋巴結(jié)轉(zhuǎn)移等臨床病理參數(shù)之間的相關(guān)性,初步探討其在直腸癌發(fā)生發(fā)展中的臨床意義,以期為闡明直腸癌的發(fā)病機(jī)制提供理論依據(jù)。方法:收集南昌大學(xué)第一附屬醫(yī)院胃腸外科2013年6月至2014年4月手術(shù)切除后經(jīng)病理證實為直腸癌的石蠟包埋組織標(biāo)本60例,并取相應(yīng)癌旁直腸組織和手術(shù)切除后經(jīng)病理診斷為直腸腺瘤組織標(biāo)本各20例作為對照組,采用鏈霉菌抗生物素蛋白-過氧化物酶連結(jié)法(streptavidin-perosidase,SP法)檢測直腸癌組織、直腸腺瘤組織及癌旁正常直腸組織中PrPC的表達(dá)情況,分別統(tǒng)計各組PrPC陽性表達(dá)率,應(yīng)用統(tǒng)計分析軟件SPSS19.0分析處理實驗數(shù)據(jù)。結(jié)果:PrPC在直腸癌組織中的陽性表達(dá)率最高,為65%(39/60),高于直腸腺瘤組的35%(7/20)及癌旁正常直腸組的15%(3/20)(P0.05),但直腸腺瘤組與癌旁正常直腸組PrPC陽性表達(dá)率比較差異無顯著性(P0.05),腫瘤分化程度為中/高分化組PrPC陽性率為54.76%(23/42),腫瘤分化程度為低分化組PrPC陽性表達(dá)率為88.89%(16/18),兩組差異比較具有統(tǒng)計學(xué)意義(P0.05);TNM分期為I-II期PrPC陽性表達(dá)率為45.45%(10/22),TNM分期為III-IV期PrPC陽性表達(dá)率為76.32%(29/38),兩組PrPC陽性表達(dá)率比較差異有統(tǒng)計學(xué)意義(P0.05);直腸癌淋巴結(jié)有轉(zhuǎn)移組PrPC陽性表達(dá)率為77.78%(28/36),無淋巴結(jié)轉(zhuǎn)移組PrPC陽性表達(dá)率為45.83%(11/24),兩組比較差異有統(tǒng)計學(xué)意義(P0.05);然而PrPC在不同性別、年齡、腫瘤位置及腫瘤大小中的表達(dá)差異均無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:PrPC在直腸癌組織中的表達(dá)高于直腸腺癌及癌旁直腸組織中的表達(dá),提示PrPC與直腸癌的發(fā)生發(fā)展密切相關(guān);PrPC的表達(dá)與直腸癌的分化程度、TNM分期及淋巴結(jié)轉(zhuǎn)移顯著相關(guān),提示PrPC可能參與了直腸癌的發(fā)展、浸潤及轉(zhuǎn)移。
[Abstract]:Objective: to detect the expression of cellular prion protein (PrPC) in rectal cancer tissues, rectal adenoma tissues and adjacent normal rectal tissues, and to analyze the expression level and differentiation of PrPC in rectal cancer. To explore the clinical significance of clinical staging and lymph node metastasis in the occurrence and development of rectal cancer. Methods: from June 2013 to April 2014, 60 cases of paraffin embedded tissues of rectal cancer proved by pathology were collected from gastrointestinal surgery of the first affiliated Hospital of Nanchang University. The rectal tissues adjacent to cancer and 20 cases of rectal adenoma diagnosed by pathology after resection were taken as the control group. Streptavidin-perosidase SP method was used to detect rectal cancer tissue by streptavidin-perosidase (SP) method. The expression of PrPC in rectal adenoma and adjacent normal rectal tissues was analyzed. The positive expression rate of PrPC in each group was counted, and the experimental data were analyzed by SPSS19.0 software. Results the positive rate of PrPC was the highest in rectal cancer. It was 65 / 39 / 60, higher than 35 / 7 / 20 in rectal adenoma and 15 / 3 / 20 / 20 in normal rectum adjacent to carcinoma, but there was no significant difference in PrPC positive rate between rectal adenoma group and paracancerous normal rectum group. The positive rate of PrPC was 54.76% / 2342% in middle / high differentiation group, but there was no significant difference between rectal adenoma group and paracancerous normal rectum group (P 0.05). The positive expression rate of PrPC in the low differentiation group was 88.89 / 18 / 18. The difference between the two groups was statistically significant. The positive expression rate of PrPC in I-II stage was 45.4545 / 22. The positive expression rate of PrPC in III-IV stage was 76.32 / 29 / 3838, and the difference between the two groups was significant. The positive expression rate of PrPC in lymph node metastasis group of rectal cancer was 77.78% / 36%, and the positive rate of PrPC expression in non-lymph node metastasis group was 45.83% / 24%. The difference between the two groups was statistically significant (P 0.05). However, PrPC was expressed in different genders. There was no significant difference in age, tumor location and tumor size. Conclusion the expression of PrPC in rectal cancer is higher than that in rectal adenocarcinoma and adjacent rectal tissue. The results suggest that the expression of PrPC is closely related to the occurrence and development of rectal cancer, and the expression of PrPC is significantly correlated with the differentiation and lymph node metastasis of rectal cancer, suggesting that PrPC may be involved in the development, invasion and metastasis of rectal cancer.
