聲輻射力脈沖成像技術(shù)對乳腺惡性腫物的診斷價值

【摘要】:目的探討聲輻射力脈沖成像(ARFI)技術(shù)對乳腺惡性腫瘤的診斷價值。方法對127例患者130個乳腺腫塊進(jìn)行常規(guī)超聲及ARFI檢查,記錄聲觸診組織成像(VTI)與二維超聲圖像病灶面積比值(AR)及聲觸診組織量化(VTQ)技術(shù)所測量的病灶聲觸診剪切波速度(SWV),與術(shù)后病理對照,分析乳腺良惡性的組間差異。結(jié)果 VTQ對惡性乳腺腫塊診斷價值的ROC曲線下面積為0.90(95%CI:0.86~0.96)。以SWV=2.9m/s為VTQ診斷界點(diǎn)診斷乳腺惡性腫瘤的靈敏度、特異度、準(zhǔn)確度及陽性預(yù)測值分別為93.2%、71.9%、83.8%和81.0%。AR對惡性乳腺腫塊診斷價值的ROC曲線下面積為0.84(95%CI:0.78~0.91)。以1.27為診斷界點(diǎn),判斷乳腺惡性腫瘤的靈敏度、特異度、準(zhǔn)確度及陽性預(yù)測值分別為76.7%、82.5%、79.2%和84.8%。將VTQ(以2.9m/s為診斷界值)和AR(1.27為診斷界值)并聯(lián)診斷乳腺惡性腫瘤的靈敏度、特異度、準(zhǔn)確度及陽性預(yù)測值分別為97.3%、61.4%、81.5%和76.3%。結(jié)論 ARFI有助于乳腺腫塊良惡性的鑒別診斷,VTQ診斷乳腺惡性腫塊靈敏度優(yōu)于VTI,二者聯(lián)合應(yīng)用可使超聲診斷乳腺惡性腫塊的靈敏度進(jìn)一步提高。
[Abstract]:Objective to evaluate the diagnostic value of acoustic power pulse imaging (ARFI) in breast malignant tumors. Methods 130 breast masses in 127 patients were examined by conventional ultrasound and ARFI. The acoustic palpation shear wave velocities measured by acoustic palpation tissue imaging (VTI) and two-dimensional ultrasound image area ratio (ARR) and acoustic palpation tissue quantization (VTQQ) technique were compared with postoperative pathology. Results the area under the ROC curve of VTQ for the diagnosis of malignant breast masses was 0.9095% CI: 0.86 0.960.The sensitivity and specificity of SWV=2.9m/s as the diagnostic threshold for breast malignant tumors were analyzed. The accuracy and positive predictive value were 93.2% and 81.0%, respectively. The area under the ROC curve of the diagnostic value of AR for malignant breast masses was 0.84 ~ 95% CI: 0.780.91. Taking 1.27 as the diagnostic threshold, the sensitivity and specificity of breast malignant tumors were judged. The accuracy and positive predictive value were 79.2% and 84.8%, respectively. The sensitivity and specificity of VTQ (taking 2.9 m / s as diagnostic threshold) and AR(1.27 in parallel for diagnosis of breast malignant tumors were studied. The accuracy and positive predictive value of VTQ were 97.3% and 76.33.Conclusion the sensitivity of ARFI in differential diagnosis of benign and malignant breast masses is better than that of VTI.The combined use of the two methods can further improve the sensitivity of ultrasonography in the diagnosis of breast malignant masses.
