【摘要】：目的:分析乳腺包被性乳頭狀癌(encapsulated papillary carcinoma,EPC)的臨床、病理特征及生存情況。方法:回顧性分析新疆醫(yī)科大學(xué)附屬腫瘤醫(yī)院2010年1月至2016年12月新疆醫(yī)科大學(xué)附屬腫瘤醫(yī)院收治的63例包被性乳頭狀癌患者的臨床資料,結(jié)合文獻(xiàn)分析患者的一般情況、臨床表現(xiàn)、病理特征、影像學(xué)表現(xiàn)、治療及預(yù)后。對比分析單純性EPC和EPC伴其他類型癌患者臨床、病理特征的區(qū)別。結(jié)果:63例EPC患者均為女性,發(fā)病年齡29-82歲,平均年齡60.3歲。影像學(xué)檢查無特征性表現(xiàn)。免疫組化中ER和PR陽性率分別為90.5%和85.7%,HER-2陽性率為4.8%。單純性EPC和EPC伴其他類型癌患者的月經(jīng)狀態(tài)、ER、PR、Ki67表達(dá)、是否發(fā)生腋窩淋巴結(jié)轉(zhuǎn)移以及分子分型差異有統(tǒng)計學(xué)意義(P≤0.05),而兩組患者的民族、年齡、腫瘤大小、HER2基因表達(dá)以及TNM分期差異無統(tǒng)計學(xué)意義(P0.05)。63例EPC患者的1年、2年、5年的無病生存率分別為98.1%、95.7%、80.9%。單純性EPC患者與EPC伴其他類型癌兩組患者的5年無病生存率分別為87.5%與71.5%,兩組患者的生存率比較差異有統(tǒng)計學(xué)差異(χ2=4.081,P=0.043)。結(jié)論:包被性乳頭狀癌好發(fā)于老年女性患者,預(yù)后較好,5年無病生存率為80.9%。單純性EPC預(yù)后較EPC伴其他類型癌預(yù)后好。單純性EPC與EPC伴其他類型癌比較患者的月經(jīng)狀態(tài)、ER、PR、Ki67表達(dá)以及是否發(fā)生腋窩淋巴結(jié)轉(zhuǎn)移有差異。關(guān)于EPC的治療方式以及預(yù)后特點仍需長時間大樣本的隨訪觀察。
[Abstract]:Objective: to analyze the clinical features of papillary carcinoma of the breast (EPC). Methods: the clinical data of 63 patients with wrapped papillary carcinoma admitted from January 2010 to December 2016 in the Cancer Hospital affiliated to Xinjiang Medical University were retrospectively analyzed. Combined with literature analysis, the general situation, clinical manifestations, pathological features, imaging manifestations, treatment and prognosis of patients with simple EPC and EPC with other types of cancer were compared and analyzed. Results 63 cases of EPC were all female. The age of onset was 29-82 years with an average age of 60.3 years. The positive rates of ER and PR in immunohistochemical staining were 90.5% and 85.7%, respectively. The positive rates of ER and PR in patients with simple EPC and EPC accompanied with other types of carcinoma were 4.8%.
