緊密連接相關(guān)蛋白在肺癌腦轉(zhuǎn)移瘤中的表達(dá)及臨床意義

【摘要】:目的:探討血-腦屏障Claudin-5、Occludin及Zo-1蛋白在肺癌腦轉(zhuǎn)移瘤組織和正常腦組織中的表達(dá)差異,并分析其臨床意義。方法:應(yīng)用Envision免疫組化法,檢測Claudin-5、Occludin及Zo-1在63例肺癌腦轉(zhuǎn)移瘤組織和28例正常腦組織中的表達(dá)情況,研究三種緊密連接蛋白在轉(zhuǎn)移瘤組織和正常組織中的不同表達(dá),并應(yīng)用SPSS軟件,分析轉(zhuǎn)移瘤中三種緊密連接蛋白表達(dá)水平與患者各種臨床病理因素(性別、年齡、轉(zhuǎn)移灶大小及個數(shù)、病理分型)、生存時間的關(guān)系。結(jié)果:1.Claudin-5蛋白主要表達(dá)于細(xì)胞膜中,其在正常腦組織、腦轉(zhuǎn)移瘤組織中的高表達(dá)率各為50%和25.4%,差異有統(tǒng)計學(xué)意義(χ2=5.310,P=0.021)。Claudin-5在肺癌腦轉(zhuǎn)移瘤病灶中的表達(dá)與患者的年齡及轉(zhuǎn)移灶個數(shù)有關(guān),差異有統(tǒng)計學(xué)意義,而與患者性別、腫塊直徑、原發(fā)灶病理學(xué)分型無統(tǒng)計學(xué)相關(guān)性。2.Occludin蛋白主要表達(dá)在細(xì)胞膜中,在正常腦組織、轉(zhuǎn)移瘤組織中的高表達(dá)率各為57.1%、34.9%,差異有統(tǒng)計學(xué)意義(χ2=3.936,P=0.047)。腦轉(zhuǎn)移瘤組織中Occludin的表達(dá)與患者的年齡及轉(zhuǎn)移灶個數(shù)有關(guān),與患者性別、腫塊直徑、原發(fā)灶病理學(xué)分型無關(guān),差別無統(tǒng)計學(xué)意義(P0.05)。3.胞質(zhì)附屬蛋白Zo-1在正常腦組織、轉(zhuǎn)移瘤組織中的高表達(dá)率各為75%、33.3%,兩者高表達(dá)率的差異有統(tǒng)計學(xué)意義(χ2=13.542,P0.001)。肺癌腦轉(zhuǎn)移瘤組織中Zo-1的表達(dá)與患者的年齡及轉(zhuǎn)移灶個數(shù)有關(guān)(P0.05),差別有統(tǒng)計學(xué)意義,而與患者性別、腫塊直徑、原發(fā)灶病理學(xué)分型無明顯相關(guān)性。4.Claudin-5、Occludin及Zo-1蛋白在高表達(dá)組的mOS較低表達(dá)組的mOS長,差別有統(tǒng)計學(xué)意義(P0.05)。5.采用Spearman相關(guān)分析提示,Claudin-5、Occludin及Zo-1在肺癌腦轉(zhuǎn)移瘤組織中的表達(dá),兩兩之間呈正相關(guān)(P0.05)。結(jié)論:與正常腦組織相比,血-腦屏障上的Claudin-5、Occludin及Zo-1蛋白在腦轉(zhuǎn)移瘤組織中呈低表達(dá),表達(dá)水平的高低與患者的生存時間有關(guān),提示緊密連接相關(guān)蛋白Claudin-5、Occludin、Zo-1表達(dá)下調(diào)在肺癌腦轉(zhuǎn)移瘤的發(fā)生發(fā)展中可能起著一定的作用。
[Abstract]:Objective: to investigate the expression of Claudin-5 Occludin and Zo-1 protein in brain metastases of lung cancer and normal brain tissues. Methods: Envision immunohistochemical method was used to detect Claudin-5. The expression of Occludin and Zo-1 in 63 cases of lung cancer brain metastases and 28 cases of normal brain tissues were studied. SPSS software was used to analyze the expression level of three kinds of tight junction protein in metastatic tumor and the clinicopathological factors (sex, age, size and number of metastatic foci, pathological classification). Results 1. Claudin-5 protein was mainly expressed in cell membrane. The high expression rates of Claudin-5 protein in normal brain tissues and brain metastases were 50% and 25.4% respectively. The expression of Claudin-5 in brain metastases of lung cancer was related to the age of patients and the number of metastatic foci. The difference was statistically significant, but there was no statistical correlation with sex, tumor diameter and pathological classification of primary lesion. 2.Occludin protein was mainly expressed in the cell membrane and normal brain tissue. The high expression rates in metastatic tumor tissues were 57.1 and 34.9respectively, the difference being statistically significant (蠂 2 / 3.936). The expression of Occludin in brain metastases was related to the age of patients and the number of metastatic foci, but not to sex, tumor diameter and pathological type of primary tumor. The high expression rate of cytoplasmic accessory protein Zo-1 in normal brain tissue and metastatic tumor tissue was 75% and 33 3% respectively. The difference of high expression rate between the two groups was statistically significant (蠂 2 = 13.542). The expression of Zo-1 in brain metastases of lung cancer was related to the age of patients and the number of metastatic foci. The difference was statistically significant, but it was related to the sex of patients and the diameter of tumor. There was no significant correlation between pathological classification of primary lesions. 4. Claudin-5 Occludin and Zo-1 protein were longer in mOS with high expression than those with low expression. The difference was statistically significant (P 0.05). Spearman correlation analysis suggested that Claudin-5. The expression of Occludin and Zo-1 in brain metastases of lung cancer was positively correlated with that of normal brain tissue (P 0.05). The expression of Claudin-5 cludin and Zo-1 protein in the blood-brain barrier was low in the brain metastases, and the expression level was related to the survival time of the patients. It is suggested that the down-regulation of the tight junction associated protein Claudin-5 Occludingnan Zo-1 may play a role in the development of brain metastases of lung cancer.
