【摘要】：本課題來源:總后勤部“十五”攻關(guān)課題資助項目No.01Z100�？傉n題名稱:特殊環(huán)境下戰(zhàn)創(chuàng)傷護理技術(shù)與裝備研究。 背景:我軍把長江以南的東南沿海省域,如浙江、福建、廣東、廣西、海南、臺灣以及江蘇南部、云南南部和西南部海拔在1500m以下的谷地劃為熱帶地區(qū),屬濕熱氣候。這一特殊環(huán)境下,創(chuàng)傷感染率高,應(yīng)激刺激強度大,機體損傷程度重。隨著新時期國際局勢的變化及我軍戰(zhàn)略重點的轉(zhuǎn)移,高溫高濕環(huán)境下戰(zhàn)創(chuàng)傷的救治與護理成為我軍醫(yī)療護理工作的重點。此外,我們所處的廣東地區(qū)長夏暖冬,夏季炎熱多濕,太陽輻射強,平均氣溫28℃,極端氣溫38℃～41℃,相對濕度可達85%～98%。尤其是南海諸島,四周環(huán)海,處于典型的濕熱海洋氣候,空氣中的相對濕度高達100%,常年氣溫30℃以上多達8個月,而且本地區(qū)經(jīng)濟發(fā)達,工傷交通事故多。因此,加強高溫高濕環(huán)境下戰(zhàn)創(chuàng)傷的救護技術(shù)研究有重要的軍事和社會意義。 高溫、高濕復(fù)合因素不僅增加了機體的代謝消耗,較易發(fā)生內(nèi)環(huán)境紊亂,還降低了機體在創(chuàng)傷后的應(yīng)激和抵抗能力,使戰(zhàn)創(chuàng)傷的傷情更加嚴(yán)重和復(fù)雜。另外,由于高溫造成體內(nèi)水分大量丟失,在傷后失血的基礎(chǔ)上,更加重了循環(huán)血容量的不足,造成全身各臟器灌注不足。因此,有必要及時給予補液和有效降溫措施。而目前現(xiàn)有的降溫措施存在著降溫效果不穩(wěn)定(如酒精擦浴、冰帽)、體積大,不易攜帶(如降溫機)或是不易操作(如體外血液冷卻法、腹腔低溫
