【摘要】：目的了解銀川市2013—2014年大氣污染物濃度與人群暴露水平,定量分析和評(píng)價(jià)該市大氣污染物與出生缺陷的暴露-反應(yīng)關(guān)系及其關(guān)鍵窗口期。方法收集銀川市2013年10月1日—2014年12月31日出生缺陷個(gè)案資料,以及該市2013年1月1日—2014年12月31日空氣質(zhì)量數(shù)據(jù)、氣象資料。采用病例交叉設(shè)計(jì)分析大氣污染物濃度與出生缺陷的暴露-反應(yīng)關(guān)系。結(jié)果研究期間銀川市大氣PM2.5年均濃度為51μg/m~3,PM10年均濃度為114μg/m~3,SO_2年平均濃度為73μg/m~3,NO_2年平均濃度為43μg/m~3,均高于GB 3095—2012《環(huán)境空氣質(zhì)量標(biāo)準(zhǔn)》國家二級(jí)標(biāo)準(zhǔn)限值。在控制氣象因素的影響下,妊娠后前3個(gè)月大氣PM2.5、PM10、SO_2濃度每升高10μg/m~3,可能會(huì)增加出生缺陷的發(fā)生風(fēng)險(xiǎn),OR(95%CI)分別為1.017(1.013~1.022),1.011(1.009~1.014),1.002(1.001~1.003);分娩前第1個(gè)月NO_2、CO濃度每升高10μg/m~3可能會(huì)增加出生缺陷的風(fēng)險(xiǎn),OR(95%CI)分別為1.003(1.001~1.006),1.106(1.018~1.202);分娩前第2個(gè)月NO_2每升高10μg/m~3可能會(huì)增加出生缺陷的風(fēng)險(xiǎn),OR(95%CI)為1.008(1.005~1.011);分娩前3個(gè)月大氣PM2.5、PM10、NO_2、CO濃度每升高10μg/m~3可能會(huì)增加出生缺陷的風(fēng)險(xiǎn),OR(95%CI)分別為1.008(1.005~1.011),1.013(1.010~1.015),1.012(1.009~1.015),1.208(1.106~1.320),均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論銀川市大氣污染較重,妊娠早期是PM2.5、PM10、SO_2對(duì)出生缺陷影響的關(guān)鍵窗口期,妊娠晚期是NO_2、CO對(duì)出生缺陷影響的關(guān)鍵窗口期。
[Abstract]:Objective to investigate the concentration of air pollutants and population exposure level in Yinchuan City from 2013 to 2014, and to quantitatively analyze and evaluate the exposure-response relationship between air pollutants and birth defects in Yinchuan City and its key window period. Methods the data of birth defects in Yinchuan City from October 1, 2013 to December 31, 2014, as well as the air quality data and meteorological data from January 1, 2013 to December 31, 2014 were collected. The relationship between air pollutant concentration and exposure-response to birth defects was analyzed by case cross design.
