【摘要】：目的:總結(jié)食用小龍蝦所致的橫紋肌溶解癥的臨床特點(diǎn)、治療及預(yù)后。方法:回顧分析本院2016年7月6日至8月26日急診科收治的84例因進(jìn)食小龍蝦所致橫紋肌溶解病例,觀察該病的臨床特點(diǎn)、治療及預(yù)后。結(jié)果:所有患者發(fā)病前40 min~24 h均有食用小龍蝦史,主要表現(xiàn)為不同程度的肌肉疼痛、肢體無(wú)力、呼吸困難及尿色改變等。實(shí)驗(yàn)室檢測(cè):磷酸肌酸激酶(6 105.49±466.51)U·L~(-1),正常值20~200 U·L~(-1);乳酸脫氫酶(1 080.76±114.80)U·L~(-1),正常值330~445 U·L~(-1);肌紅蛋白(571.13±29.38)ng·ml~(-1),正常值25 ng·ml~(-1);所有患者的血肌酐、尿素氮和血鉀均在正常范圍,部分患者伴有白細(xì)胞總數(shù)及中性粒細(xì)胞比例升高;經(jīng)堿化尿液、休息、水化、飲水及防治并發(fā)癥等治療,所有患者均痊愈出院。結(jié)論:哈夫病目前病因、發(fā)病機(jī)制均不明了,臨床表現(xiàn)多樣。針對(duì)食用小龍蝦所致橫紋肌溶解患者,及時(shí)就診、早期診斷及治療,預(yù)后較好。
[Abstract]:Objective: to summarize the clinical features, treatment and prognosis of rhabdomyolysis caused by crayfish. Methods: the clinical features, treatment and prognosis of 84 cases of rhabdomyolysis caused by eating crayfish were retrospectively analyzed in the emergency department of our hospital from July 6 to August 26, 2016. Results: all the patients had a history of eating crayfish at 40 min~24 before onset of disease. The main manifestations were muscle pain, limb weakness, dyspnea and urinary color change. Laboratory tests: creatine phosphokinase (6 105.49 鹵466.51) U L ~ (-1), lactate dehydrogenase (1 080.76 鹵114.80) U L ~ (-1) and lactate dehydrogenase (1 080.76 鹵114.80) U L ~ (-1). Myoglobin (571.13 鹵29.38) ng ml~ (-1), normal value 25 ng ml~ (-1), serum creatinine, urea nitrogen and serum potassium in all patients were in the normal range, and some patients were accompanied by the increase in the total number of white blood cells and the proportion of neutrophilic granulocytes. After alkali urine, rest, hydration, drinking water and prevention and treatment of complications, all patients were cured and discharged. Conclusion: the etiology and pathogenesis of Haff's disease are not clear at present, and the clinical manifestations are diverse. For the patients with rhabdomyolysis caused by eating crayfish, early diagnosis and treatment have good prognosis.
