【摘要】：目的分析國家重大公共衛(wèi)生服務(wù)項目育齡婦女乳腺癌、宮頸癌(以下簡稱兩癌)篩查覆蓋水平及影響因素。方法利用吉林省2013年衛(wèi)生服務(wù)調(diào)查數(shù)據(jù),共納入15~64歲女性5 496人。計算育齡婦女兩癌篩查覆蓋率,并采用logistic回歸對兩癌篩查率的影響因素進行多因素分析。結(jié)果 2013年吉林省宮頸癌和乳腺癌篩查率僅為11.9%和14.7%,其中城市篩查率為15.13%和18.81%高于農(nóng)村篩查率7.47%和9.09%。25~54歲年齡段兩癌篩查率最高,宮頸癌和乳腺癌篩查率分別為16.27%和19.38%。多因素分析結(jié)果顯示,中專以上學(xué)歷、在校學(xué)生、婚姻狀況和收入為兩癌篩查的影響因素(P0.05)。距離最近醫(yī)療機構(gòu)距離、自評健康得分、5年內(nèi)分娩與兩癌篩查覆蓋率均不相關(guān)(P0.05)。結(jié)論吉林省兩癌篩查利用水平較低,城鄉(xiāng)差異較大。促進兩癌篩查全面覆蓋,應(yīng)關(guān)注加強醫(yī)保與公共衛(wèi)生的整合銜接,健全各級醫(yī)療機構(gòu)協(xié)作機制,并提供有效的健康教育。
[Abstract]:Objective to analyze the coverage level and influencing factors of breast cancer and cervical cancer screening in women of childbearing age. Methods A total of 5 496 women aged 15 ~ 64 years were included in the health service survey data of Jilin province in 2013. The coverage rate of two cancer screening in women of childbearing age was calculated, and the factors influencing the screening rate of two cancers were analyzed by logistic regression. Results in 2013, the screening rates of cervical cancer and breast cancer in Jilin Province were only 11.9% and 14.70.The screening rates in cities were 15.13% and 18.81% respectively, which were higher than those in rural areas (7.47% and 9.09.2554). The screening rates of cervical cancer and breast cancer were 16.27% and 19.38%, respectively. The results of multivariate analysis showed that secondary school education, school students, marital status and income were the influencing factors of two cancer screening (P0.05). Distance from the nearest medical institution, self-rated health score, delivery within 5 years and the coverage rate of two cancer screening were not related (P0.05). Conclusion the level of cancer screening and utilization in Jilin Province is low and the difference between urban and rural areas is great. To promote the comprehensive coverage of cancer screening, we should pay attention to strengthening the integration of medical insurance and public health, improve the cooperation mechanism of medical institutions at all levels, and provide effective health education.
