【摘要】：目的分析河北省手足口病(hand foot and mouth disease,HFMD)的流行特征及病原學(xué)構(gòu)成,為手足口病的防控提供科學(xué)依據(jù)。方法利用描述流行病學(xué)方法對(duì)河北省2011-2015年疾病監(jiān)測(cè)信息報(bào)告系統(tǒng)上報(bào)的手足口病病例和實(shí)驗(yàn)室檢測(cè)結(jié)果進(jìn)行統(tǒng)計(jì)分析。結(jié)果 2011-2015年累計(jì)報(bào)告手足口病病例349 113例,年均發(fā)病率為95.91/10萬,其中2012年和2014年報(bào)告發(fā)病率最高,呈現(xiàn)隔年高水平流行的現(xiàn)象。高發(fā)期集中在5~7月,10~12月間又出現(xiàn)一個(gè)次高峰或"拖尾"峰,患者主要為≤5歲兒童,尤其以1~3歲組嬰幼兒為主。各地區(qū)發(fā)病水平存在差異,以發(fā)病率和重癥率為分類指標(biāo)對(duì)各地市進(jìn)行聚類分析和賦值評(píng)價(jià),得到重點(diǎn)防控地區(qū)為廊坊市、滄州市、衡水市、石家莊市、保定市和承德市。不同年份病原構(gòu)成亦不同,在2013年和2015年其他腸道病毒分別取代柯薩奇病毒A組16型(coxsachievirus A16,Cox A16)、腸道病毒71型(enterovirus 71,EV71)成為當(dāng)年主要病原體,構(gòu)成比高達(dá)44.89%和66.23%。結(jié)論河北省手足口病呈現(xiàn)明顯的季節(jié)、人群以及一定的地區(qū)分布規(guī)律,應(yīng)進(jìn)一步關(guān)注手足口病病原譜的變化,加強(qiáng)重點(diǎn)地區(qū)防控。
