【摘要】：目的探討影響我國出生缺陷發(fā)生的主要危險(xiǎn)因素,為制定最佳預(yù)防控制決策、實(shí)行有效的干預(yù)措施提供科學(xué)依據(jù)。方法利用Meta分析方法綜合分析國內(nèi)關(guān)于出生缺陷發(fā)病危險(xiǎn)因素的研究文獻(xiàn)21篇,累計(jì)病例6 112例,對(duì)照15 741例。據(jù)納入與排除標(biāo)準(zhǔn)進(jìn)行篩選,由異質(zhì)性檢驗(yàn)結(jié)果決定采用固定或隨機(jī)效應(yīng)模型,并對(duì)合并結(jié)果進(jìn)行χ2檢驗(yàn)。結(jié)果單因素分析的主要合并結(jié)果 OR值為:先天畸形家族史36.22,孕早期患感染性疾病5.62,孕早期接觸化學(xué)制劑4.19,父親吸煙3.89,孕期接觸鎮(zhèn)靜藥3.19,妊娠并發(fā)癥2.94,孕期多食高蛋白食物0.37;多因素分析的主要合并結(jié)果OR值為:孕早期感染病毒7.65,孕期精神狀態(tài)差5.44,孕早期發(fā)熱4.70,妊娠早期接觸化學(xué)毒物3.90,流產(chǎn)史3.59,孕期補(bǔ)充復(fù)合維生素0.45。結(jié)論影響我國出生缺陷發(fā)生的主要危險(xiǎn)因素為先天畸形家族史、孕早期患感染性疾病、孕早期接觸化學(xué)制劑、父親吸煙、妊娠并發(fā)癥;出生缺陷的保護(hù)因素為孕期多食高蛋白食物、孕期補(bǔ)充復(fù)合維生素和服用葉酸。
[Abstract]:Objective to explore the main risk factors affecting the occurrence of birth defects in China, in order to make the best decision of prevention and control. Methods 21 literatures about risk factors of birth defects were analyzed by Meta analysis, and 6 112 cases were accumulated. 15 741 cases were screened according to inclusion and exclusion criteria, and fixed or random effect models were selected from the results of heterogeneity test. Results OR value of the main combined results of univariate analysis was 36.22 family history of congenital malformation, 5.62 of infective diseases in early pregnancy, 4.19 of exposure to chemical preparations in early pregnancy, 3.89 of father smoking and contact with sedation during pregnancy. Drug 3.19, pregnancy complications 2.94, high protein food 0.37. The main results of multivariate analysis were as follows: early pregnancy infection virus 7.65, mental state poor during pregnancy 5.44, fever 4.70 in early pregnancy, exposure to chemical poison 3.90 in early pregnancy, history of abortion. Conclusion the main risk factor for birth defects in China is the family history of congenital malformation. Infective diseases in early pregnancy, exposure to chemicals in early pregnancy, smoking by fathers, complications of pregnancy, protective factors of birth defects were high protein diet during pregnancy, multivitamin supplementation and folic acid administration during pregnancy.
