OCT檢測(cè)視神經(jīng)纖維層厚度與視野的相關(guān)性研究

【摘要】: 目的:研究第三代光學(xué)相干斷層掃描儀( stratus optical coherence tomography 3000,OCT3 )檢測(cè)原發(fā)性開(kāi)角型青光眼(primary open angle glaucoma,POAG)視神經(jīng)纖維層( retinal nerve fiber layer,RNFL)厚度的改變和與OCULUS Easyflied視野計(jì)檢測(cè)的視野平均缺損(mean defect,MD)的相關(guān)性,評(píng)估OCT3檢測(cè)視神經(jīng)纖維層厚度在POAG早期診斷中的價(jià)值。方法:34例( 68眼)正常人、37例(68眼)可疑開(kāi)角型青光眼(SOAG)、37例(68眼)原發(fā)性開(kāi)角型青光眼(POAG) ,采用OCULUS Easyflied視野計(jì)和OCT3分別進(jìn)行視野平均缺損和視神經(jīng)纖維層厚度的檢測(cè)。比較OCT3檢測(cè)三組間視神經(jīng)纖維層厚度的差異,分析原發(fā)性開(kāi)角型青光眼組自動(dòng)視野計(jì)檢測(cè)的視野平均缺損(MD)與OCT3檢測(cè)的視神經(jīng)纖維層厚度(RNFL)的關(guān)系。結(jié)果:OCT3檢測(cè)正常組、SOAG組、POAG組三組中的任意兩組的視神經(jīng)纖維層厚度,除正常組與原發(fā)性可疑型青光眼組的Tavg無(wú)顯著性差異(P=0.072)外,余三組間兩兩比較各值均存在顯著性差異( P 0.05)。在原發(fā)性開(kāi)角型青光眼組, Avg. Thi、Iavg和Savg與MD的相關(guān)性較好( P 0.01),Tavg與MD的相關(guān)性次之(P=0.010),Navg與MD的相關(guān)性較差(P=0.052)。結(jié)論:OCT3能夠檢測(cè)到青光眼早期的視神經(jīng)纖維層厚度的改變,且與MD有基本一致的較好相關(guān)性。OCT3檢測(cè)視神經(jīng)纖維層厚度可用于POAG的早期診斷。
[Abstract]:Objective: to study the detection of optic nerve fiber layer (retinal nerve fiber layer, in primary open angle glaucoma (primary open angle glaucoma,POAG) by the third generation optical coherence tomography (stratus optical coherence tomography 3000, Oct 3). The correlation between the change of RNFL) thickness and the average visual field defect (mean defect,MD) detected by OCULUS Easyflied myometer was used to evaluate the value of OCT3 in the early diagnosis of POAG.
