【摘要】：目的探討20G和23G聯(lián)合玻璃體切割術(shù)治療玻璃體視網(wǎng)膜疾病的臨床療效。方法本研究為前瞻性臨床研究,將首次行玻璃體切割術(shù)的玻璃體視網(wǎng)膜疾病患者99例99眼納入研究,利用隨機(jī)數(shù)字表法將患眼隨機(jī)分為傳統(tǒng)20G玻璃體切割組(A組)、經(jīng)結(jié)膜20G玻璃體切割組(B組)、20G和23G聯(lián)合玻璃體切割組(C組),每組各33眼。觀察各組患者行玻璃體切割術(shù)的手術(shù)時(shí)間,術(shù)后1 d、3 d、7 d術(shù)眼的舒適度,術(shù)后1周的矯正視力、眼壓及并發(fā)癥等情況。結(jié)果各組手術(shù)前后矯正視力比較,差異有統(tǒng)計(jì)學(xué)意義(均為P=0.000),各組之間手術(shù)前后視力差異無(wú)統(tǒng)計(jì)學(xué)意義(均為P0.05)。各組手術(shù)時(shí)間比較,A、B、C各組的手術(shù)時(shí)間分別為80~135 min、65~115 min、64~100 min,A組與B、C兩組之間比較,差異有顯著統(tǒng)計(jì)學(xué)意義(P=0.003、0.002)。B、C兩組之間進(jìn)行比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.12)。各組患者術(shù)后不同時(shí)間點(diǎn)術(shù)眼舒適度進(jìn)行比較,A組與B、C兩組分別比較,差異均有顯著統(tǒng)計(jì)學(xué)意義(均為P0.01),B、C兩組之間進(jìn)行比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(均為P0.05)。術(shù)后1周內(nèi),患者的眼壓為(14.25±3.18)mmHg(1 kPa=7.5 mmHg),術(shù)后不同時(shí)間點(diǎn),各組患者無(wú)一例發(fā)生低眼壓等并發(fā)癥。術(shù)后并發(fā)癥各組之間比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(均為P0.05)。結(jié)論 20G和23G聯(lián)合玻璃體切割手術(shù),手術(shù)安全高效,操作簡(jiǎn)便易行,易于推廣和使用。
[Abstract]:Objective to investigate the clinical effect of 20g and 23G combined with vitrectomy in the treatment of vitreoretinal diseases. Methods 99 cases (99 eyes) of vitreoretinal diseases who underwent vitrectomy for the first time were included in the prospective clinical study. The eyes were randomly divided into traditional 20G vitrectomy group (group A) by random digital table method. There were 33 eyes in each group through conjunctival 20G vitrectomy group (group B) and 20G and 23G combined vitrectomy group (group C). The operation time of vitrectomy, the comfort of eyes 1 day, 3 days and 7 days after operation, the corrected visual acuity, intraocular pressure and complications 1 week after operation were observed.
