【摘要】：目的:評估人工耳蝸再次植入術(shù)的臨床特點,總結(jié)人工耳蝸再次植入手術(shù)的臨床經(jīng)驗。方法:回顧性研究分析2002-2012年間進行的25例人工耳蝸再次植入術(shù)患者的資料。主要分析再次植入手術(shù)的原因、術(shù)中的發(fā)現(xiàn)、術(shù)中電極植入的數(shù)量及再次植入術(shù)前后的聽力及言語的表現(xiàn)。結(jié)果:本次再次植入術(shù)的原因中,8例植入體物理故障,8例為電極位置不佳,4例首次植入術(shù)后效果不佳,3例皮瓣感染導(dǎo)致植入體暴露,1例顳骨病變,1例術(shù)后面神經(jīng)刺激征。25例患者均在我科成功進行人工耳蝸再植入術(shù),術(shù)后患者原并發(fā)癥均治愈,聽覺行為分級標(biāo)準(zhǔn)和言語可懂度分級標(biāo)準(zhǔn)及圖文識別軟件測試言語識別率顯著提高,無嚴重并發(fā)癥發(fā)生。結(jié)論:人工耳蝸再植入者原因具有多樣性,需根據(jù)不同的原因進行個性化的分析和處理。人工耳蝸再次植入術(shù)可以取得較好的臨床及聽力效果,而且是一種安全和有效的手術(shù)方式。
[Abstract]:Objective: to evaluate the clinical characteristics of cochlear reimplantation and summarize the clinical experience of cochlear reimplantation. Methods: the data of 25 patients undergoing cochlear reimplantation from 2002 to 2012 were studied and analyzed retrospectively. the data of 25 patients undergoing cochlear reimplantation from 2002 to 2012 were analyzed retrospectively. This paper mainly analyzes the causes of reimplantation, the findings during the operation, the number of electrodes implanted during the operation and the hearing and speech manifestations before and after the reimplantation. Results: there were 8 cases of physical failure of implantation, 8 cases of poor electrode position, 4 cases of poor effect after the first implantation, 3 cases of skin flap infection leading to implantation exposure and 1 case of temporal bone lesion. 1 case of nerve stimulation sign after operation. All 25 patients underwent cochlear implantation successfully in our department, and the original complications were cured after operation. The speech recognition rate of auditory behavior classification standard, speech intellibility classification standard and graphic and text recognition software test was significantly improved, and no serious complications occurred. Conclusion: the causes of cochlear reimplantation are diverse and need to be analyzed and treated according to different reasons. Cochlear implantation can achieve good clinical and hearing results, and it is a safe and effective method.
