【摘要】：目的:系統(tǒng)評價(jià)大劑量甲基強(qiáng)的松龍沖擊療法與安慰劑治療急性視神經(jīng)炎的臨床療效和安全性。方法:檢索Medline、OVID和中文科技期刊全文數(shù)據(jù)庫、維普中文科技期刊數(shù)據(jù)庫,檢索時(shí)間截至2009年5月。按照文內(nèi)所述納入標(biāo)準(zhǔn)收集大劑量靜脈應(yīng)用激素沖擊治療急性視神經(jīng)炎的相關(guān)研究文獻(xiàn),采用Meta分析對檢索文獻(xiàn)進(jìn)行綜合定量評價(jià)。結(jié)果:IVMP與安慰劑比較共納入3個(gè)研究433例患者,IVMP在早期顯示出相對優(yōu)于安慰劑組的有效率,但在六個(gè)月后,兩組有效率的差異無統(tǒng)計(jì)學(xué)意義。結(jié)論:早期靜脈注射大劑量甲基強(qiáng)的松龍可加快急性視神經(jīng)炎的康復(fù),六個(gè)月后與安慰劑組無明顯差別。在降低急性視神經(jīng)炎的復(fù)發(fā)率、甲基強(qiáng)的松龍的最佳用藥劑量及療程仍需開展更多設(shè)計(jì)良好、方法學(xué)質(zhì)量更高的隨機(jī)對照試驗(yàn)來證實(shí)。
[Abstract]:Objective: to evaluate the clinical efficacy and safety of high dose methylprednisolone pulse therapy and placebo in the treatment of acute optic neuropathy. Methods: Medline,OVID and Chinese sci-tech periodical full-text database and VIP Chinese sci-tech periodical database were searched up to May 2009. According to the inclusion criteria described in this paper, the related research literature of high dose intravenous hormone shock therapy for acute optic neuropathy was collected, and the retrieval literature was evaluated comprehensively and quantitatively by Meta analysis. Results: a total of 433 patients with IVMP and placebo were enrolled in the study. IVMP showed a relatively superior effective rate to the placebo group at an early stage, but after six months, there was no significant difference in the effective rate between the two groups. Conclusion: early intravenous injection of high dose methylprednisolone can accelerate the recovery of acute optic neuropathy, and there is no significant difference between the placebo group and the placebo group after six months. In order to reduce the recurrence rate of acute optic neuropathy, the best dose and course of treatment of methylprednisolone still need to be confirmed by more randomized controlled trials with good design and higher methodological quality.
