【摘要】：目的探討早期喉癌手術(shù)陰性切緣組織中PCNA、COX-2的表達(dá)與腫瘤局部復(fù)發(fā)的關(guān)系。方法選擇63例經(jīng)CO2激光手術(shù)治療后的早期喉癌患者標(biāo)本,對(duì)切除的腫瘤組織及切緣組織中PCNA、COX-2表達(dá)進(jìn)行檢測(cè),所有患者均隨訪2年以上,對(duì)其生存及腫瘤復(fù)發(fā)情況進(jìn)行觀察。結(jié)果 63例早期喉癌患者腫瘤組織中PCNA、COX-2陽(yáng)性率分別為73.02%、71.43%;切緣組織中PCNA和COX-2陽(yáng)性率分別為33.33%、30.16%。腫瘤組織與切緣組織中PCNA、COX-2均陽(yáng)性的患者分別為41例和13例,陽(yáng)性率分別為65.08%、20.63%;63例隨訪病例中,局部復(fù)發(fā)17例,復(fù)發(fā)率為26.98%。PCNA陽(yáng)性患者復(fù)發(fā)率為71.43%,COX-2陽(yáng)性患者復(fù)發(fā)率為73.68%(P0.05)。聯(lián)合檢測(cè)PCNA和COX-2陽(yáng)性者復(fù)發(fā)率明顯高于單獨(dú)陽(yáng)性者(P0.05)。結(jié)論早期喉癌手術(shù)切緣組織中PCNA、COX-2的表達(dá)是喉癌患者預(yù)后的重要生物學(xué)標(biāo)志,并可根據(jù)檢查結(jié)果制定針對(duì)性的后續(xù)治療方案,聯(lián)合檢測(cè)二者對(duì)術(shù)后治療具有更可靠的指導(dǎo)意義。
[Abstract]:Objective to investigate the relationship between the expression of PCNA,COX-2 in negative margin of early laryngeal carcinoma and local recurrence of laryngeal carcinoma. Methods the expression of PCNA,COX-2 was detected in 63 cases of early laryngeal carcinoma treated by CO2 laser. All the patients were followed up for more than 2 years. The survival and tumor recurrence were observed. Results the positive rates of PCNA,COX-2 and COX-2 in 63 patients with early laryngeal carcinoma were 73.02% and 41.43% respectively, and those in incised margin tissues were 33.33% and 30.16%, respectively. The positive rate of PCNA,COX-2 was 65.08% and 20.63% in 41 cases and 13 cases of tumor tissue and incisor margin tissue, respectively, and the positive rate was 65.08% and 20.63%, respectively. The recurrence rate of 26.98%.PCNA positive patients was 71.43%, and that of Cox-2 positive patients was 73.68% (P0.05). The recurrence rate of PCNA and COX-2 positive group was significantly higher than that of single positive group (P 0.05). Conclusion the expression of PCNA,COX-2 in the surgical margin of early laryngeal carcinoma is an important biological marker for prognosis of laryngeal carcinoma, and it can be used as a follow-up therapy according to the results of examination. Combined detection has more reliable guiding significance for postoperative treatment.
