【摘要】：目的評(píng)價(jià)α-2b干擾素( Interferonα-2b , IFNα-2b )與5-氟尿嘧啶(5-Fluorouracil ,5-Fu)用于青光眼濾過(guò)術(shù)的抗瘢痕效果。 方法采用隨機(jī)對(duì)照法,將臨床收治的52例(56只眼)初次手術(shù)的青光眼患者分為IFNα-2b組(28眼)與5-Fu組(28眼)。4例為同一患者左右眼對(duì)照。均行小梁切除術(shù)。前組用含50萬(wàn)U /mlIFNα-2b,后組用濃度12.5 mg/ml 5-Fu,均術(shù)中一次性使用,共4 min。術(shù)后根據(jù)情況結(jié)膜下注藥并針刺分離,記錄患者眼壓、視力、并發(fā)癥等情況。隨訪時(shí)間4個(gè)月。 結(jié)果兩組眼壓術(shù)后與術(shù)前相比均顯著降低(P㩳0.001);5-Fu組術(shù)后視力下降者11只眼(39.3%),而IFNα-2b組3只眼(10.7%)。兩組間差異有統(tǒng)計(jì)學(xué)意義(2=6.095,P㩳0.05);IFNα-2b組形成的濾過(guò)泡以II型為主,5-Fu組則以I型為主(χ2=8.096,P㩳0.05 );IFNα-2b組角膜上皮損傷程度明顯輕于5-Fu組(P㩳0.05)。 結(jié)論IFNα-2b與5-Fu應(yīng)用于青光眼濾過(guò)手術(shù)中均能阻止術(shù)后濾過(guò)通道瘢痕化,降低手術(shù)失敗率,且IFNα-2b副作用更小,是抗青光眼手術(shù)的理想用藥。
[Abstract]:Objective to evaluate the anti-scar effect of Interferon 偽 -2b (IFN 偽 -2b) and 5-Fluorouracil (5-Fu) in glaucoma filtration surgery. Methods Fifty-two patients (56 eyes) with primary glaucoma were randomly divided into IFN 偽 -2b group (28 eyes) and 5-Fu group (28 eyes). All patients underwent trabeculectomy. The former group was given 500000 U / mlIFN 偽 -2b, and the latter group was given 12.5 mg/ml 5-Fu.All of them were used once during operation for a total of 4 min.. The intraocular pressure, visual acuity and complications were recorded according to the condition of conjunctival injection and acupuncture separation. The follow-up time was 4 months. Results the postoperative IOP in both groups was significantly lower than that before operation (P0. 001), the visual acuity of 11 eyes (39. 3%) in 5-Fu group and 3 eyes (10. 7%) in IFN 偽-2b group. There was a significant difference between the two groups (2: 6.095P0. 05); IFN 偽 -2b group). The main type of bleb was II type, while that of 5-Fu group was I type (蠂 2 + 8.096%). The degree of corneal epithelial injury in IFN 偽-2b group was significantly less than that in 5-Fu group (P0. 05). Conclusion both IFN 偽 -2b and 5-Fu can prevent the scarring of postoperative filtering channels and reduce the failure rate of operation, and IFN 偽 -2b is an ideal drug for glaucoma surgery.
