【摘要】：目的探討腺樣體肥大并分泌性中耳炎的患兒在腺樣體切除術(shù)后其分泌性中耳炎預(yù)后的影響因素。方法以198例(252耳)腺樣體肥大伴分泌性中耳炎患兒為研究對(duì)象,其中15例(23耳)行鼓膜置管術(shù)者不計(jì)入統(tǒng)計(jì)結(jié)果。對(duì)其余183例(229耳)住院行鼻內(nèi)鏡下腺樣體切除術(shù)的患兒,隨訪3~12個(gè)月,比較不同性別、病程、腺樣體肥大程度、咽鼓管功能及是否伴慢性鼻-鼻竇炎患兒的療效。結(jié)果 183例(229耳)并分泌性中耳炎患兒行腺樣體切除術(shù)后分泌性中耳炎治愈143例(157耳),好轉(zhuǎn)20例(39耳),無效20例(33耳)。病程長(zhǎng)、腺樣體Ⅲ度肥大、咽鼓管功能未恢復(fù)、伴慢性鼻-鼻竇炎的患兒分別較病程短、腺樣體Ⅰ、Ⅱ度大、咽鼓管功能恢復(fù)及不伴慢性鼻-鼻竇炎患兒療效差(P0.05)。結(jié)論病程較長(zhǎng)、腺樣體Ⅲ度肥大、咽鼓管功能障礙、伴慢性鼻-鼻竇炎可能影響腺樣體肥大并分泌性中耳炎患兒在腺樣體切除術(shù)后其分泌性中耳炎的預(yù)后。
[Abstract]:Objective to investigate the prognostic factors of secretory otitis media in children with adenoid hypertrophy and secretory otitis media after adenoidectomy. Methods 198 cases (252 ears) of adenoid hypertrophy with secretory otitis media were studied, of which 15 cases (23 ears) underwent tympanic membrane catheterization. The remaining 183 cases (229 ears) who underwent endoscopic adenoidectomy were followed up for 3 ~ 12 months. The results were compared between different sexes, course of disease, degree of adenoid hypertrophy, eustachian tube function and whether children with chronic rhinosinusitis. Results in 183 cases (229 ears) with secretory otitis media, 143 cases (157 ears) were cured after adenoidectomy, 20 cases (39 ears) improved, 20 cases (33 ears) failed. The course of disease was long, adenoid third degree hypertrophy, eustachian tube function did not recover, the children with chronic rhinosinusitis had shorter course of disease, adenoid 鈪，
