凋亡抑制因子XIAP、Livin在喉癌中的表達(dá)和意義

【摘要】： 背景:喉癌(laryngeal carcinoma、cancer of the larynx、laryngeal cancer、Larynx cancer、carcinoma of larynx)是頭頸部常見的惡性腫瘤,好發(fā)年齡為40～60歲,男性較女性多見,本癌的發(fā)生與吸煙、酗酒、胃食管反流、營養(yǎng)不良、乳頭狀瘤病毒感染、種族、性別、年齡、免疫系統(tǒng)功能減退、長期吸入有害物質(zhì)及發(fā)聲過度等因素有關(guān)。多年來,在喉全切除后行放射治療是喉癌的標(biāo)準(zhǔn)治療�；颊呷砬谐委熀�,發(fā)聲功能完全喪失,也可能發(fā)生吞咽功能障礙,導(dǎo)致在許多方面生活質(zhì)量下降,包括營養(yǎng),社會功能和個人衛(wèi)生。此外,放化療綜合治療和單純放療組有比全喉切除術(shù)組在生存率控制的勝算低。所以,我們迫切需要找到治療喉癌的新途徑。 目的:探討凋亡抑制蛋白XIAP(X-染色體連鎖蛋白)和Livin在喉癌組織中的表達(dá)及與病人的性別、年齡、臨床分期、淋巴結(jié)轉(zhuǎn)移和分化程度的相關(guān)性。方法:從107例2007年1月-2009年12月收治所有確診為喉癌患者中,抽取42例喉癌及癌旁組織切片作對照,術(shù)前均未進(jìn)行放療及化療。應(yīng)用免疫組織化學(xué)二步法測定其XIAP、Livin蛋白的表達(dá)情況。 結(jié)果: 1、XIAP在42例喉癌患者中陽性表達(dá)率為100%,在癌旁正常組織中陽性表達(dá)率為61.9%,兩組間比較差異有統(tǒng)計學(xué)意義(P0.01);Livin在42例喉癌患者中陽性表達(dá)率為42.86%,在癌旁正常組織中陽性表達(dá)率為9.52%,兩組間比較差異有統(tǒng)計學(xué)意義(P0.01) 2、在42例喉癌中,XIAP和Livin的陽性表達(dá)率在臨床分期兩組間比較差異有統(tǒng)計學(xué)意義(P0.01); 3、在42例喉癌患者中,XIAP和Livin的陽性表達(dá)率在性別、年齡、有無頸部淋巴結(jié)轉(zhuǎn)移和分化程度各兩組間比較,差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:凋亡抑制蛋白XIAP和Livin在喉癌的發(fā)生發(fā)展中起重要作用,但和性別、年齡、有無頸部淋巴結(jié)轉(zhuǎn)移和分化程度差異無統(tǒng)計學(xué)意義。
[Abstract]:Background: laryngeal carcinoma (laryngeal carcinoma,cancer of the larynx,laryngeal cancer,Larynx cancer,carcinoma of larynx) is a common malignant tumor in the head and neck. It is more common in men than in women. The incidence of this cancer is associated with smoking, alcoholism, gastroesophageal reflux and malnutrition. Papillomavirus infection, race, sex, age, hypofunction of immune system, long-term inhalation of harmful substances and hypervocal factors. Radiotherapy after total laryngectomy has been the standard treatment for laryngeal cancer for many years. After total laryngectomy, the vocal function is completely lost and the dysphagia may occur, leading to a decline in the quality of life in many areas, including nutrition, social function and personal hygiene. In addition, radiotherapy and chemotherapy combined therapy and radiotherapy alone have lower survival control than total laryngectomy. Therefore, we urgently need to find a new way to treat laryngeal cancer. Objective: to investigate the expression of XIAP and Livin in laryngeal carcinoma and their correlation with sex, age, clinical stage, lymph node metastasis and differentiation. Methods: from January 2007 to December 2009, 42 cases of laryngeal carcinoma and paracancerous tissue sections were selected from 107 patients with laryngeal cancer. No radiotherapy and chemotherapy were performed before operation. The expression of XIAP,Livin protein was detected by immunohistochemical two-step method. Results: 1the positive expression rate of XIAP was 100 in 42 patients with laryngeal carcinoma and 61.9 in the adjacent normal tissues. The difference between the two groups was statistically significant (P0.01). The positive expression rate of Livin was 42.86 in 42 cases of laryngeal carcinoma and 9.52 in the adjacent normal tissue. The difference between the two groups was statistically significant (P0.01) 2, and in 42 cases of laryngeal carcinoma. The positive expression rate of XIAP and Livin was significantly different between the two groups in clinical stage (P0.01). 3. In 42 patients with laryngeal carcinoma, the positive expression rates of XIAP and Livin in sex, age, cervical lymph node metastasis and differentiation were not significantly different between the two groups (P0.05). Conclusion: XIAP and Livin play an important role in the occurrence and development of laryngeal carcinoma, but there is no significant difference in sex, age, cervical lymph node metastasis and differentiation.
