【摘要】：目的:探討23G微創(chuàng)玻璃體切除手術(shù)治療球形視網(wǎng)膜脫離的療效及其安全性。方法:2010年1月至2013年12月在我院就診的36例(36眼)球形視網(wǎng)膜脫離病例,接受23G微創(chuàng)玻璃體切除手術(shù),術(shù)后隨訪裂隙燈、眼壓、最佳矯正視力、間接檢眼鏡檢查和并發(fā)癥發(fā)生情況。結(jié)果:隨訪時(shí)間9~18(12.7±2.1)個(gè)月,術(shù)后視網(wǎng)膜解剖復(fù)位率97.2%(35/36眼),1眼在術(shù)后視網(wǎng)膜未完全復(fù)位,2周后下方發(fā)現(xiàn)新裂孔,行鞏膜外扣帶術(shù)后視網(wǎng)膜復(fù)位。末次隨訪最佳矯正視力顯著高于術(shù)前,除術(shù)中手術(shù)切口滲漏縫合6眼(16.7%),術(shù)后一過性眼壓升高9眼(25.0%),前房少量積血1眼(2.8%),晶狀體羽毛狀混濁5眼(13.9%)外,未出現(xiàn)醫(yī)源性裂孔、脈絡(luò)膜脫離、眼內(nèi)炎等嚴(yán)重并發(fā)癥。結(jié)論:23G微創(chuàng)玻璃體切除術(shù)治療球形視網(wǎng)膜脫離是安全有效的。
[Abstract]:Objective: to evaluate the efficacy and safety of 23 G minimally invasive vitrectomy in the treatment of spherical retinal detachment. Methods: from January 2010 to December 2013, 36 patients (36 eyes) with spherical retinal detachment underwent 23 G minimally invasive vitrectomy, followed up by slit lamp, intraocular pressure, and best corrected visual acuity. Indirect ophthalmoscope examination and complications. Results: the follow-up time was 9 months (12.7 鹵2.1) months. The anatomical reattachment rate of retina was 97.2% (35 / 36 eyes). Retinal reattachment was performed after scleral buckling. The best corrected visual acuity at the last follow-up was significantly higher than that before operation, except for 6 eyes (16.7%) with operative incision leakage and suture, 9 eyes (25.0%) with postoperative transient intraocular pressure elevation, and 1 eye (2.8%) with a small amount of hyphema in the anterior chamber. There were no serious complications such as iatrogenic hiatus, choroidal detachment and endophthalmitis in 5 eyes (13.9%) of lens feather opacity. Conclusion: 23 G minimally invasive vitrectomy is safe and effective in the treatment of spherical retinal detachment.
