已治愈單眼弱視患者多焦視覺誘發(fā)電位的研究

【摘要】:目的評價已治愈的單眼弱視患者多焦視覺誘發(fā)電位(multifocal visual evoked potential, mfVEP)的恢復(fù)情況。 方法記錄32例6~15歲已治愈的單眼弱視(斜視性弱視、屈光參差性弱視或斜視性弱視合并屈光參差性弱視)患者及30例7~17歲正常人多焦視覺誘發(fā)電位,分別比較已治愈單眼弱視患者弱視眼、對側(cè)眼及正常人三組之間上、下半視野不同離心度(1環(huán)、2環(huán)、3環(huán)、4環(huán))多焦視覺誘發(fā)電位特征峰P波的潛伏期及振幅密度。 結(jié)果弱視眼上、下半視野1環(huán)、2環(huán)、3環(huán)、4環(huán)特征峰P波潛伏期及振幅密度較對側(cè)眼差別均無統(tǒng)計學(xué)意義(P0.05);弱視眼和對側(cè)眼上、下半視野1環(huán)、2環(huán)、3環(huán)、4環(huán)的特征峰P波潛伏期均較正常組顯著延長,差別有統(tǒng)計學(xué)意義(P0.05),而振幅密度較正常對照組差別無統(tǒng)計學(xué)意義(P0.05)。 結(jié)論1.已治愈單眼弱視患者,隨著視力恢復(fù)至正常水平,弱視眼的上、下半視野四個離心度mfVEP特征峰P波潛伏期和振幅密度可恢復(fù)至與對側(cè)眼相當(dāng)水平。 2.已治愈單眼弱視患者弱視眼和對側(cè)眼的上、下半視野四個離心度mfVEP特征峰P波潛伏期和振幅密度雖然無顯著差別,但弱視眼、對側(cè)眼分別與正常人比較時,它們的潛伏期仍較正常人延長,說明已治愈單眼弱視患者弱視眼和對側(cè)眼的傳導(dǎo)通路及視皮層仍存在異常。 3.已治愈單眼弱視患者弱視眼和對側(cè)眼的上、下半視野四個離心度mfVEP特征峰P波振幅密度與正常人比較,三組之間的差別無統(tǒng)計學(xué)意義,表明已治愈的單眼弱視患者的mfVEP的振幅密度較潛伏期提前恢復(fù)至正常人水平。
[Abstract]:Objective to evaluate the recovery of multifocal visual evoked potential (multifocal visual evoked potential, MF VEP) in patients with monocular amblyopia. Methods 32 cases of amblyopia (strabismus amblyopia, anisometropic amblyopia or strabismus amblyopia with anisometropia amblyopia) and 30 normal subjects aged 7 to 17 years were studied. The latency and amplitude density of the characteristic P wave of multifocal visual evoked potential in amblyopic eyes, contralateral eyes and normal subjects were compared. Results in amblyopic eyes, there was no significant difference in P wave latency and amplitude density between the eyes with amblyopia (P 0.05), the amblyopia eyes and the contralateral eyes (P < 0.05), and the P wave latency and amplitude density of the P wave were not significantly different between the eyes with amblyopia and those with the contralateral eye. The latency of P wave in the lower half field of 1 ring, 2 ring, 3 ring and 4 ring were significantly longer than that in the normal group (P0.05), but the amplitude density had no statistical significance compared with the normal control group (P0.05). Conclusion 1. With the recovery of visual acuity to the normal level, the latency and amplitude density of the four characteristic peak P waves in the lower half visual field of the amblyopic eyes recovered to the same level as the contralateral eyes. 2. In the amblyopia and contralateral eyes of the cured monocular amblyopia, the latency and amplitude density of the P wave of the four eccentric mfVEP characteristic peaks in the lower half field were not significantly different, but the amblyopic eyes and the contralateral eyes were compared with the normal eyes respectively. Their latent period is still longer than that of normal people, indicating that the conduction pathway and visual cortex of amblyopia and contralateral eye are still abnormal in patients with monocular amblyopia. 3. In the amblyopia and contralateral eyes of the cured monocular amblyopia patients, there was no significant difference between the three groups in the P wave amplitude density of the four eccentric mfVEP characteristic peaks in the lower half field compared with the normal subjects. The results showed that the amplitude density of mfVEP in the cured monocular amblyopia patients returned to normal level earlier than the latent period.
