【摘要】：目的探討青光眼小梁切除術(shù)(TRABE)后功能性濾過泡形態(tài)與眼表改變的關(guān)系。方法選擇2012年6月至2013年6月該院行TRABE治療的86例青光眼患者,根據(jù)術(shù)后眼表改變情況分組,其中加重組24例,無變化組62例。分析兩組患者術(shù)后整術(shù)眼眼表改變情況,分析濾過泡面積、夾角眼部感覺改變情況的關(guān)系。結(jié)果兩組患者術(shù)后的平均眼壓均顯著低于術(shù)前(均P0.05)。隨訪6個月,薄壁濾過泡計56眼,厚壁濾過泡計30眼。術(shù)后患者的角結(jié)膜熒光素染色(FSS)評分、虎紅染色(RBSS)評分、淚膜儀分級、對稱指數(shù)(SRI)、不規(guī)則指數(shù)(SAI)均顯著高于術(shù)前,潛在視力(PUV)及淚膜破裂時間(BUT)均顯著低于術(shù)前(均P0.05)。手術(shù)前后比較基礎(chǔ)淚液分泌試驗(Schirmer)情況差異無統(tǒng)計學(xué)意義(P0.05)。與無變化組相比,加重組眼部感覺改變的患者濾過泡面積差異無統(tǒng)計學(xué)意義(P0.05)。加重組有異物感、畏光感、視力波動以及睜眼困難的患者濾過泡夾角均顯著小于無變化組(均P0.05)。根據(jù)Spearman法分析可知,上述指標(biāo)均與濾過泡夾角呈顯著負(fù)相關(guān)(r=-0.615,-0.589,-0.712,-0.698,P=0.000,0.002,0.000,0.000)。與無變化組比較,加重組有干澀感和有癢感的患者濾過泡夾角差異均無統(tǒng)計學(xué)意義(均P0.05)。結(jié)論青光眼患者接受TRABE之后,恢復(fù)效果較好,但加重組的有異物感、畏光感、視力波動以及睜眼困難的患者濾過泡夾角更小,因此應(yīng)重視其濾過泡角度情況。
[Abstract]:Objective to investigate the relationship between functional bleb morphology and ocular surface changes after trabeculectomy. Methods 86 patients with glaucoma treated with TRABE from June 2012 to June 2013 were divided into two groups according to the changes of ocular surface, including 24 cases with recombination and 62 cases with no change. The changes of ocular surface, the area of filtering bleb and the angle of eye sensation were analyzed. Results the mean IOP in both groups was significantly lower than that before operation (P 0.05). Follow-up for 6 months showed 56 eyes with thin-wall bleb meter and 30 eyes with thick wall bleb meter. The FSS score, red staining score, lacrimal film grading, symmetry index (SRI), irregular index (SAI) were significantly higher in postoperative patients than those before operation, and the latent visual acuity (PUV) and tear film rupture time (but) were significantly lower than those before operation (all P 0.05). Before and after operation, there was no significant difference of Schirmer in basic tear secretion test (P 0.05). Compared with no change group, there was no significant difference in filtration surface product in patients with recombined ocular sensory changes (P 0.05). The angle of bleb in the patients with foreign body sensation, photophobia, visual fluctuation and difficulty in opening eyes was significantly lower than that in the no change group (all P 0.05). According to Spearman method, the above indexes were negatively correlated with the angle of filtration bleb, r-0.615 ~ (-0.589) ~ -0.712 ~ (-1) -0.698 ~ 0.0002 ~ 0.000 ~ 0.000 ~ 0.000 ~ 0.000 ~ 0.000 ~ 0.000 ~ 0.000 ~ 0.000 ~ 0.000 ~ 0.000 ~ 0.000 ~ 0.000 ~ 0.000 ~ 0. Compared with no change group, there was no significant difference in filtration bleb angle between the patients with dry and itchy recombination (all P 0.05). Conclusion the recovery effect of glaucoma patients after receiving TRABE is better, but the patients with foreign body sensation, photophobia, visual fluctuation and difficulty in opening eyes have smaller filtering bleb angle, so we should pay attention to the filtering bleb angle.
