【摘要】：目的分析預(yù)防性玻璃體切割術(shù)(prophylactic vitrectomy,PPV)治療早期急性視網(wǎng)膜壞死綜合征(acute retinal necrosis,ARN)的遠(yuǎn)期療效。方法收集47例(55眼)行PPV治療的早期不伴視網(wǎng)膜脫離ARN患者。眼底熒光血管造影診斷明確后行一期玻璃體切割伴硅油(49眼)或氣體(6眼)填充術(shù)。治療后隨訪12~88個(gè)月,平均30個(gè)月,分析患者術(shù)前、一期術(shù)后2周及末次隨訪時(shí)最佳矯正視力、視網(wǎng)膜復(fù)位、并發(fā)癥等情況。結(jié)果術(shù)前最佳矯正視力≥0.1者4眼,一期術(shù)后2周≥0.1者27眼,未次隨訪≥0.1者23眼。平均手術(shù)次數(shù)1.93次,手術(shù)原因?yàn)閱渭児栌腿〕?7眼,二次硅油取出2眼,因增生性玻璃體視網(wǎng)膜病變所致視網(wǎng)膜脫離行硅油置換術(shù)1眼,裂孔性視網(wǎng)膜脫離行硅油置換術(shù)1眼,并發(fā)白內(nèi)障1眼。末次隨訪視網(wǎng)膜在位55眼。結(jié)論早期PPV治療ARN遠(yuǎn)期療效穩(wěn)定。遠(yuǎn)期隨訪對(duì)于評(píng)價(jià)ARN預(yù)后至關(guān)重要。
[Abstract]:Objective to analyze the long term effect of prophylactic vitrectomy for early acute retinal necrosis syndrome (ARN). Methods 47 cases (55 eyes) of ARN without retinal detachment were treated with PPV. Fundus fluorescein angiography (FFA) was performed in 49 eyes with vitrectomy and silicone oil (49 eyes) or with gas filling (6 eyes). The patients were followed up for 12 ~ 88 months (mean 30 months). The best corrected visual acuity, retinal reattachment and complications were analyzed before, 2 weeks and the last follow-up. Results the preoperative best corrected visual acuity (BCVA) was better in 4 eyes than 0.1 in 4 eyes, 27 eyes in 2 weeks after primary operation with 鈮，
