【摘要】：目的：探討病灶內(nèi)注射平陽霉素治療眼眶低流速靜脈畸形的療效與安全性，總結(jié)臨床經(jīng)驗，進而指導臨床工作。 方法：回顧性分析2002年－2013年間上海交通大學醫(yī)學院附屬第九人民醫(yī)院眼科的33例(33眼)行病灶內(nèi)平陽霉素注射治療的眼眶低流速靜脈畸形患者臨床資料，記錄病變部位、初次接受該治療的年齡、治療次數(shù)、劑量、隨訪時間，比較治療前后情況，包括病變體積、青紫程度、高出皮面情況等；評估與平陽霉素相關副作用的發(fā)生情況。 結(jié)果：本項目眼眶低流速靜脈畸形患者的年齡范圍4-45歲(平均23.8±11.2歲)，其中男性18例(54.5%)，女性15例(45.5%)；其中17例（51.5%）位于上、下眼瞼、9例(27.3%)累及結(jié)膜、4例(12.1%)位于肌錐內(nèi)、2例(6.1%)位于眉弓、1例(3.0%)位于內(nèi)眥部。隨訪時間2-48月（平均7.9±8.2月）�；颊卟≡顑�(nèi)注射平陽霉素平均次數(shù)為2±1次；術前病灶平體積為4.4cm3，術后為1.0cm3(t=4.63, P 0.001),體積平均縮小84.4%(28%-100%);94%的患者病變體積顯著縮小，其中，淺表型病變顯著縮小的比例為94.7%(18/19)，深部病變100%(3/3)，，混合病變90.9%(10/11)。術后患者外觀包括病變青紫程度及高出皮面厚度較術前均有顯著改善，隨訪未發(fā)現(xiàn)復發(fā)病例。治療相關的副作用有術后短期內(nèi)局部結(jié)膜水腫及局部皮下萎縮。 結(jié)論：病灶內(nèi)注射平陽霉素可作為眼眶低流速靜脈畸形一線治療方法
[Abstract]:Objective: to investigate the efficacy and safety of intrafocal injection of pingyangmycin in the treatment of orbital low-flow venous malformation. Methods: the clinical data of 33 cases (33 eyes) of orbital low velocity venous malformation treated with pingyangmycin injection from 2002 to 2013 were retrospectively analyzed, and the location of the lesions was recorded. The age, times, dosage and follow-up time of the treatment were compared before and after treatment, including volume of lesion, degree of cyanotic color, higher appearance of skin, etc. The occurrence of side effects associated with pingyangmycin was evaluated. Results: the age range of patients with orbital low flow venous malformation was 4 to 45 years old (mean 23.8 鹵11.2 years old). 9 cases of inferior eyelid palpebrae were involved in conjunctiva in 4 cases (12.1) and 2 cases were located in musculocones, 2 cases were located in the middle of musculocones, 1 case was located in the middle part of the medial canthus, 1 case was located in the arch of the eyebrow, and 1 case was located in the middle canthus. The follow-up time was 2 to 48 months (mean 7.9 鹵8.2 months). The mean intratumoral injection of pingyangmycin was 2 鹵1, and the mean volume of pingyangmycin was 4.4cm ~ (-3) before operation and 1.0cm ~ (-3) after operation (P 0.001). The mean volume of pingyangmycin was reduced by 84.4% -100% (P < 0.05). The proportion of superficial lesions significantly reduced was 94.7 / 18 / 19, deep lesions 100 / 3 / 3, mixed lesions 90.9% / 10 / 11%. The appearance of the patients, including the degree of cyanotic lesions and the thickness of the skin, were significantly improved compared with those before operation, and no recurrence cases were found at the follow-up. Treatment-related side effects include local conjunctival edema and subcutaneous atrophy in the short term after operation. Conclusion: injection of pingyangmycin into the focus can be used as a first-line method for the treatment of orbital low velocity venous malformation.
