【摘要】： 眩暈是耳鼻咽喉-頭頸外科常見(jiàn)的臨床癥狀之一,隨著人口老齡化的進(jìn)展,本癥發(fā)病率在日漸增高,受到國(guó)內(nèi)外醫(yī)務(wù)界廣泛重視。近年來(lái),隨著眩暈的基礎(chǔ)及臨床研究日益深入,各種前庭功能檢查儀器應(yīng)運(yùn)而生,大大豐富了眩暈診斷和治療的內(nèi)涵,使良性陣發(fā)性位置性眩暈等已由癥狀診斷發(fā)展為有特定名稱、病理改變、診斷標(biāo)準(zhǔn)和系統(tǒng)內(nèi)外科治療的獨(dú)立疾病。 良性陣發(fā)性體位性眩暈是最常見(jiàn)的周?chē)匝?其治療目前國(guó)際上主要采用手法復(fù)位治療,其確切的療效已被許多文獻(xiàn)所驗(yàn)證,但仍有一些未解決的問(wèn)題。本文的目的在于觀察手法復(fù)位治療良性陣發(fā)性位置性眩暈的療效,探討手法復(fù)位治療良性陣發(fā)性位置性眩暈的機(jī)制和效果。 本文收集2009年5月-2010年3月因眩暈就診于吉林大學(xué)第一醫(yī)院耳鼻咽喉-頭頸外科,經(jīng)前庭功能檢查(眼震視圖描記法的變位性眼震試驗(yàn))結(jié)合患者病史確診為良性陣發(fā)性位置性眩暈的23例患者,行手法復(fù)位1個(gè)月后隨訪所有病例,痊愈19例(82.61%),其中15例(78.95%)在首次治療后即無(wú)復(fù)發(fā),其余4例(21.05%)經(jīng)間隔3～5天的2～3次手法復(fù)位痊愈;好轉(zhuǎn)4例(17.39%);總有效率100%。痊愈的19例在1個(gè)月內(nèi)隨訪中未曾復(fù)發(fā)。結(jié)果證明:手法復(fù)位治療BPPV,大部分患者眩暈及眼震立即消失,安全且療效顯著,并可重復(fù)進(jìn)行,無(wú)嚴(yán)重并發(fā)癥,是其首選治療方法。
[Abstract]:Vertigo is one of the common clinical symptoms in otolaryngal-head and neck surgery.In recent years, with the deepening of the basic and clinical studies of vertigo, various vestibular function examination instruments have emerged, which greatly enriched the connotation of diagnosis and treatment of vertigo.Benign paroxysmal positional vertigo has evolved from symptomatic diagnosis to independent diseases with specific names pathological changes diagnostic criteria and systemic internal and surgical treatment.Benign paroxysmal positional vertigo is the most common peripheral vertigo. At present, the treatment of benign paroxysmal positional vertigo is mainly treated by manual reduction in the world. Its exact effect has been verified by many literatures, but there are still some unsolved problems.The purpose of this paper is to observe the efficacy of manual reduction in the treatment of benign paroxysmal positional vertigo, and to explore the mechanism and effect of manual reduction in the treatment of benign paroxysmal positional vertigo.From May 2009 to March 2010, she was admitted to the Department of Otorhinolaryngology and head and neck surgery in the first Hospital of Jilin University for vertigo.23 patients with benign paroxysmal positional vertigo were diagnosed by vestibular function examination (transposition nystagmus test by nystagmus view method). All the patients were followed up after one month of manual reduction.The 19 cured patients did not recur during a 1 month follow-up.The results showed that manual reduction was the first choice for the treatment of BPPV.The vertigo and nystagmus of most of the patients disappeared immediately and were safe and effective and could be repeated without serious complications.
