【摘要】： 本研究通過(guò)比較雙眼混合植入多焦點(diǎn)人工晶狀體及雙眼植入單焦點(diǎn)人工晶狀體術(shù)后視功能及視覺質(zhì)量的狀態(tài),評(píng)價(jià)混合植入多焦點(diǎn)人工晶狀體術(shù)后臨床效果及應(yīng)用價(jià)值。 研究對(duì)象選擇在吉大二院眼科醫(yī)院因雙眼白內(nèi)障行雙眼超聲乳化白內(nèi)障吸出聯(lián)合人工晶狀體植入術(shù)的患者40例,其中10例根據(jù)患者需求混合搭配植入兩種不同折射原理的多焦點(diǎn)人工晶狀體,余30例雙眼植入單焦點(diǎn)人工晶狀體,觀察術(shù)后12個(gè)月不同光照條件下的遠(yuǎn)、中、近距離的裸眼及矯正視力,立體視銳度,影像不等檢查,雙眼及單眼遠(yuǎn)距對(duì)比敏感度測(cè)定,焦點(diǎn)深度及離焦曲線的測(cè)定,閱讀速度的檢測(cè)及視覺質(zhì)量問卷調(diào)查。 通過(guò)隨訪,術(shù)后12個(gè)月多焦組患者裸眼全程視力表現(xiàn)優(yōu)異,與單焦組矯正后視力相當(dāng)。兩組立體視及不等象方面,多焦組表現(xiàn)較好。多焦組遠(yuǎn)距對(duì)比敏感度比各單眼多焦點(diǎn)人工晶狀體略高,但略低于單焦組患者,并無(wú)統(tǒng)計(jì)學(xué)意義。多焦組具有較寬泛的焦點(diǎn)深度,比各單眼多焦點(diǎn)人工晶狀體焦點(diǎn)深度更寬。閱讀速度的比較來(lái)看,多焦組裸眼閱讀速度接近單焦組矯正閱讀速度。術(shù)后滿意度及脫鏡率多焦組較高,不良視覺發(fā)生率方面多焦組略高,但對(duì)日常生活并無(wú)影響。 本研究結(jié)果表明,混合植入多焦點(diǎn)人工晶狀體設(shè)計(jì)的確能夠在遠(yuǎn)、中、近全程視力上降低患者對(duì)眼鏡的依賴,在安全可靠基礎(chǔ)上實(shí)現(xiàn)患者視覺及生活質(zhì)量的提高。
[Abstract]:In this study, we compared the visual function and visual quality between binocular mixed intraocular lens implantation and single focus intraocular lens implantation, and evaluated the clinical effect and application value of mixed implantation of multi-focus intraocular lens.Participants 40 patients with binocular phacoemulsification combined with intraocular lens implantation were selected from ophthalmological hospital of Jida second Hospital.According to the needs of the patients, 10 patients were mixed with two different refraction principles of multi-focus intraocular lens implantation, and the remaining 30 eyes were implanted with single focus intraocular lens. The distance and distance under different illumination conditions were observed 12 months after operation.Close eye and corrected visual acuity, stereopsis acuity, image inequality, distance contrast sensitivity of binocular and monocular, focal depth and defocus curve, reading speed and visual quality questionnaire.After 12 months follow-up, the uncorrected visual acuity of multi-focus group was superior to that of single-focus group.In both groups, multi-focal group performed well in stereo vision and unequal vision.The far contrast sensitivity of multi-focal group was slightly higher than that of single-eye multi-focus intraocular lens, but slightly lower than that of single focus group, and there was no statistical significance.The focal depth of multi-focal group is wider than that of single-eye multi-focal intraocular lens.In comparison of reading speed, the open eye reading speed of multi-focus group was close to that of single-focus group.The degree of satisfaction and the rate of devitoscopy were higher in the multi-focal group, but had no effect on the daily life in the multi-focal group.The results of this study indicate that the mixed implantation of multi-focus intraocular lens design can reduce the patients' dependence on eyeglasses and improve their visual and quality of life on the basis of safety and reliability.
