【摘要】：目的評估經(jīng)導(dǎo)管眼動脈灌注化療術(shù)的(TOAC)治療眼內(nèi)晚期化療失敗的視網(wǎng)膜母細(xì)胞瘤的臨床療效及安全性。方法回顧性分析2009年1月—2011年4月收治的經(jīng)化療失敗的視網(wǎng)膜母細(xì)胞瘤患兒30例39只患眼行SOAI治療的臨床資料。39只患眼共行TOAC術(shù)82次,平均每眼行2.1次,隨訪時間2~28個月,平均13.6個月。術(shù)后評估腫瘤控制的有效率、并發(fā)癥及有無局部腫瘤進(jìn)展。結(jié)果 84次眼動脈插管中,82次成功插至眼動脈,技術(shù)成功率為97.6%。術(shù)后30只眼腫瘤有不同程度的縮小,有效率76.9%,其中D期、E期分別為17、13只眼,分別占同期治療眼的85.0%、68.4%;其余9只眼(9/39,23.1%)行眼球摘除。TOAC術(shù)后10只眼(10/39,25.6%)出現(xiàn)過球結(jié)膜充血,7只眼(7/39,17.9%)發(fā)生眼瞼腫脹,3只眼(3/39,7.7%)多淚,均自行恢復(fù)。全部患兒均未出現(xiàn)血流感染等嚴(yán)重全身并發(fā)癥。結(jié)論TOAC是治療眼內(nèi)晚期化療失敗的視網(wǎng)膜母細(xì)胞瘤的安全有效的治療方法。
[Abstract]:Objective to evaluate the clinical efficacy and safety of transcatheter ophthalmic arterial infusion chemotherapy (TOAC) in the treatment of retinoblastoma with late intraocular chemotherapy failure. Methods from January 2009 to April 2011, the clinical efficacy and safety of chemotherapy failed retinoblastoma were analyzed retrospectively. Clinical data of 39 eyes of 30 cases with retinoblastoma treated with SOAI. 39 eyes were treated with TOAC for 82 times. The average time was 2.1 times per eye, and the follow-up time was 2 ~ 28 months (mean 13.6 months). The effective rate of tumor control, complications and local tumor progression were evaluated after operation. Results 82 times of 84 ophthalmic arterial catheterization were successfully inserted into the ophthalmic artery. The technical success rate was 97.60.Thirty eye tumors were reduced in varying degrees, and the effective rate was 76.9. among them, 17 eyes were in stage D and 13 eyes were in stage E, respectively. 85.0% of the eyes treated in the same period accounted for 68.4%; the other 9 eyes (9% 39.23. 1) were treated with 10 / 39% 25.6% of the 10 eyes after TOAC.) there were 7 eyes with bulbar conjunctival hyperemia and 7 eyes with bulbar conjunctival congestion (739% 17.9%). Conclusion TOAC is a safe and effective method for the treatment of retinoblastoma with late intraocular chemotherapy failure.
