【摘要】： 目的:通過改良翼狀胬肉切除術(shù)中的手術(shù)操作方法,比較分析不同方法手術(shù)的治療效果。本研究用清除異常增生的筋膜、纖維增生組織及帶鞏膜密集縫合的改良方法代替?zhèn)鹘y(tǒng)的簡單方法。比較在翼狀胬肉切除聯(lián)合自體結(jié)膜移植術(shù)中使用這2種技術(shù)的臨床結(jié)果,探求如何改良手術(shù)方式以提高胬肉的治療效果,降低胬肉術(shù)后的復(fù)發(fā)率。 方法:用回顧性分析方法研究我院自2005年1月至2009年12月因翼狀胬肉住院手術(shù)的患者148例,156只眼(女68例,72眼;男80例,84眼)。全部病例排除其他眼病及相關(guān)性全身系統(tǒng)性疾病。依照采取不同手術(shù)方式分為二組:A組傳統(tǒng)翼狀胬肉切除聯(lián)合自體結(jié)膜移植術(shù)83眼(73眼原發(fā),10眼復(fù)發(fā));B組改良翼狀胬肉切除聯(lián)合自體結(jié)膜移植術(shù)73眼(65眼原發(fā),8眼復(fù)發(fā))。所有患者術(shù)后隨訪為6個月至2年。術(shù)后觀察結(jié)膜角膜修復(fù)情況、手術(shù)后并發(fā)癥及胬肉復(fù)發(fā)情況,評價其療效。統(tǒng)計學(xué)分析兩組手術(shù)方法的效果差異。 結(jié)果:兩組手術(shù)方法的術(shù)后復(fù)發(fā)率比較:傳統(tǒng)翼狀胬肉切除聯(lián)合自體結(jié)膜移植術(shù)組13眼復(fù)發(fā),復(fù)發(fā)率為15.66%;改良翼狀胬肉切除聯(lián)合自體結(jié)膜移植術(shù)組4眼復(fù)發(fā),復(fù)發(fā)率為5.48%。改良組比傳統(tǒng)組并發(fā)癥少,兩組均沒有嚴(yán)重威脅到手術(shù)患者的視功能的主要并發(fā)癥。 結(jié)論:兩組手術(shù)方法的結(jié)果比較表明,改良組較常規(guī)組復(fù)發(fā)率低,愈合效果佳而且并發(fā)癥少。雖然清除異常增生的筋膜、纖維增生組織及帶鞏膜密集縫合的技術(shù)相對難度大和耗費時間,但從美容角度及手術(shù)結(jié)果發(fā)現(xiàn)其更優(yōu)越。我們建議采用清除異常增生的筋膜、纖維增生組織及帶鞏膜密集縫合的技術(shù)治療翼狀胬肉。
[Abstract]:Objective: to compare and analyze the therapeutic effect of different methods in pterygium excision. The modified method of dense suture of fibrous hyperplasia tissue and sclera instead of the traditional simple method was used to compare the clinical results of these two techniques in pterygium excision and autoconjunctival transplantation. To explore how to improve the operation to improve the treatment of pterygium and reduce the recurrence rate of pterygium. Methods: a retrospective study of 156 eyes (68 female, 72 eyes) with pterygium in our hospital from January 2005 to December 2009 was carried out. Other ophthalmopathy and associated systemic diseases were excluded in 80 male patients. According to different surgical methods, they were divided into two groups: group A, traditional pterygium excision and autoconjunctival transplantation in 83 eyes (73 eyes). In group B, modified pterygium excision combined with autologous conjunctival transplantation was performed in 73 eyes and 65 eyes with primary recurrence. All the patients were followed up for 6 months to 2 years. The conjunctival corneal repair was observed after operation. Postoperative complications and recurrence of pterygium were evaluated. Results: the recurrence rate was 15.66 in 13 eyes of traditional pterygium resection combined with autologous conjunctival transplantation group, 4 eyes in modified pterygium resection combined with autologous conjunctival transplantation group. The recurrence rate was 5.48%. There were fewer complications in the modified group than in the traditional group, and there were no major complications that seriously threatened the visual function of the patients in both groups. Conclusion: the results of operation in the two groups showed that the recurrence rate of the modified group was lower than that of the conventional group, the healing effect was better and the complications were less. The technique of dense suture of fibrous hyperplasia tissue and sclera is relatively difficult and time-consuming, but it is more superior from the point of view of beauty and surgical results.
