【摘要】：目的分析青光眼患者復合式小梁切除術(shù)后感染性眼內(nèi)炎的臨床危因素,并提出相應的防治對策,減少其發(fā)生。方法選取2001年3月-2013年5月行復合式小梁切除術(shù)的青光眼患者1 233例,對其臨床資料進行回顧分析,對不同性別、年齡、青光眼類型、合并疾病以及治療方案患者發(fā)生眼內(nèi)炎的概率進行統(tǒng)計,并行l(wèi)ogistic回歸分析。結(jié)果 1 233例患者共發(fā)生眼內(nèi)炎14例,發(fā)生率為1.14%;高齡、合并瞼內(nèi)翻倒睫、合并糖尿病、夏季、非層流手術(shù)室手術(shù)、未進行健康教育、未限制病房探視的患者發(fā)生眼內(nèi)炎概率明顯高于不存在上述因素的患者(P0.05);經(jīng)多因素回歸分析上述高危因素均為青光眼復合式小梁切除術(shù)感染性眼內(nèi)炎的獨立危險因素(P0.05)。結(jié)論青光眼復合式小梁切除術(shù)后感染性眼內(nèi)炎的危險因素多樣,應給予針對性的預防,一旦發(fā)生感染性眼內(nèi)炎,應立即給予萬古霉素聯(lián)合玻璃體切割手術(shù)進行治療。
[Abstract]:Objective to analyze the clinical risk factors of infectious endophthalmitis in glaucoma patients after combined trabeculectomy and to put forward corresponding preventive and therapeutic measures. Methods the clinical data of 1 233 glaucoma patients who underwent combined trabeculectomy from March 2001 to May 2013 were retrospectively analyzed. Results 14 cases (1.14%) of endophthalmitis were found in 1,233 patients, and the incidence rate was 1.14%, and the incidence of endophthalmitis was 1.14%, and the incidence of endophthalmitis was 1.14%, and the incidence of endophthalmitis was 1.14%, and the incidence of endophthalmitis was 1.14%. Non-laminar flow surgery without health education, The probability of developing endophthalmitis in patients with unconfined visits was significantly higher than that in patients without the above factors, and the risk factors mentioned above were all independent risk of infectious endophthalmitis in glaucoma trabeculectomy by multivariate regression analysis. Conclusion the risk factors of infectious endophthalmitis after combined trabeculectomy are various. Prevention should be given. Once infective endophthalmitis occurs, vancomycin combined with vitrectomy should be given immediately.
