開角型青光眼中GDxVCC參數(shù)和視野指標(biāo)的相關(guān)性研究

【摘要】: 目的:研究開角型青光眼早、中、晚期GDxVCC和視野的相關(guān)性。 方法:利用Humphrey750型計(jì)算機(jī)自動(dòng)視野計(jì)和德國蔡司公司生產(chǎn)的GDxVCC,對(duì)2009年1月~2010年2月到我院就診的開角型青光眼35人(57眼)及可疑青光眼17人(20眼)進(jìn)行檢查,并根據(jù)視野的平均缺損把開角型青光眼患者分為早期組(20眼)、中期組(15眼)和晚期組(22眼)。分析可疑青光眼組、開角型青光眼早、中、晚期組,四組間患者年齡、眼壓、屈光度及C/D比值有無差異;比較四個(gè)組GDxVCC各參數(shù)(TSNIT、SA、IA、TSD、NFI)及視野各指標(biāo)(MD、PSD、VFI)間有無差異;對(duì)各組GDxVCC參數(shù)與視野指標(biāo)進(jìn)行相關(guān)分析。 結(jié)果: 1.可疑青光眼、開角型青光眼早、中、晚,四組間年齡、眼壓、屈光度差異無統(tǒng)計(jì)學(xué)意義(P0.05),C/D比值差有統(tǒng)計(jì)學(xué)意義(P=0.00),經(jīng)兩兩比較后開角型青光眼早期及中期有差異。 2.四組間GDxVCC各參數(shù)(TSNIT、SA、IA、TSD、NFI)及視野各指標(biāo)(MD、PSD、VFI)差異均有統(tǒng)計(jì)學(xué)意義(P=0.00); 3.可疑青光眼、開角型青光眼早、中、晚期GDxVCC參數(shù)與視野指標(biāo)均呈弱相關(guān)性;開角型青光眼各期視野指標(biāo)與GDxVCC參數(shù)的相關(guān)性:早期VFI>MD>PSD,中期PSD>MD>VFI,晚期MD>VFI>PSD;開角型青光眼各期GDxVCC參數(shù)與視野指標(biāo)的相關(guān)性:早期IA>SA>NFI>TSNIT>TSD,中期SA>NFI>TSNIT>IA>TSD,晚期IA>TSNIT>NFI>SA>TSD。 結(jié)論: 1.GDxVCC能夠較好測量RNFL厚度; 2.開角型青光眼早期GDxVCC參數(shù)與視野指標(biāo)的相關(guān)性較差; 3.開角型青光眼中隨著視野缺損程度的加深GDxVCC參數(shù)與視野指標(biāo)的相關(guān)系數(shù)逐漸增大; 4.開角型青光眼早、中、晚期與視野指標(biāo)相關(guān)性最好的GDxVCC參數(shù)不同,分別為IA、SA、IA; 5.開角型青光眼中與GDxVCC參數(shù)相關(guān)性最好視野指標(biāo)是MD; 6.在開角型青光眼各期TSNIT及NFI與視野的相關(guān)性相對(duì)穩(wěn)定; 7.TSNIT標(biāo)準(zhǔn)差是與視野相關(guān)性最差的GDxVCC指標(biāo); 8.GDxVCC聯(lián)合視野能夠提高開角型青光眼的早期診斷率;
[Abstract]:Objective: to study the correlation between early, middle and late GDxVCC and visual field in open angle glaucoma. Methods: using Humphrey750 computer automatic visual field meter and GDxVCC produced by Zeiss Company in Germany, 35 patients (57 eyes) with open angle glaucoma and 17 eyes (20 eyes) with suspected glaucoma were examined from January 2009 to February 2010 in our hospital. According to the mean defect of visual field, open angle glaucoma patients were divided into early group (20 eyes) and middle group (15 eyes) and late group (22 eyes). The diopter and the ratio of C / D were different, the differences of GDxVCC parameters and visual field indexes were compared, and the correlation between GDxVCC parameters and visual field index was analyzed. Results:. 1. There was no significant difference in age, intraocular pressure and diopter between the four groups in suspected glaucoma, early, middle and late open angle glaucoma. There was no significant difference in the ratio of P 0.05 / C / D between the four groups. There were significant differences in the early and middle stages of open angle glaucoma after pairwise comparison. 2. There were significant differences in the GDxVCC parameters (TSNITT) and the visual field (GDxVCC) between the four groups. 3. There was a weak correlation between the parameters of GDxVCC and visual field in the early, middle and late stage of open angle glaucoma. The correlation between the visual field parameters of open angle glaucoma and GDxVCC parameters was as follows: early VFI > MD > PSD, middle stage PSD > MD > VFI, late stage MD > VFI > PSD, and early stage IA > SA > NFI > TSNIT > TSD, middle stage > NFI > TSNIT > IA > TSD, late stage IA > TSNIT > NFI > SA > TSD > TSD. Conclusion:. 1.GDxVCC can better measure the thickness of RNFL; 2. The correlation between GDxVCC parameters and visual field index was poor in the early stage of open-angle glaucoma. 3. In open-angle glaucoma, the correlation coefficient between GDxVCC parameter and visual field index gradually increased with the deepening of visual field defect. 4. The GDxVCC parameters with the best correlation with visual field index were different in early, middle and late stage of open-angle glaucoma. 5. In open-angle glaucoma, MDD is the best visual field index correlated with GDxVCC parameters. 6. The correlation between TSNIT and NFI and visual field was relatively stable in each stage of open-angle glaucoma. 7.TSNIT standard deviation is the worst correlation with visual field GDxVCC index; 8. GDxVCC combined visual field can improve the early diagnosis rate of open-angle glaucoma.
