血清半胱氨酸白三烯對兒童阻塞性睡眠呼吸暫停低通氣綜合征的診斷價(jià)值及意義

【摘要】:目的:探討阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)患兒血清半胱氨酸白三烯(CysLTs)水平的變化及意義,為臨床診斷和治療OSAHS提供科學(xué)依據(jù)。方法:選擇OSAHS患兒41名(OSAHS組),按AHI水平將OSAHS組分為輕、中、重三組,對照組34名,所有受試者均接受多導(dǎo)睡眠儀監(jiān)測,采用競爭酶聯(lián)免疫分析法(ELASA)測定各組血清CysLTs水平,并將OSAHS組的血清CysLTs與各睡眠呼吸參數(shù)進(jìn)行相關(guān)分析。結(jié)果:(1)OSAHS組血清CysLTs水平(683.102±89.825)pg/mL高于對照組(461.985±84.951)pg/mL,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。(2)OSAHS組患兒血清CysLTs水平與呼吸暫停低通氣指數(shù)(AHI)、最長呼吸暫停時(shí)間(LAT)、最長低通氣時(shí)間(LHT)呈正相關(guān)(r值分別為0.417、0.422、0.208,P均<0.05),與最低血氧飽和度(LSaO2)水平、平均血氧飽和度(MSaO2)呈負(fù)相關(guān)(r值分別為-0.192、-0.255,P均0.05),(3)重度OSAHS組患兒血清CysLTs水平(773.118±92.015)pg/mL高于中度OSAHS組患兒血清CysLTs水平(712.144±41.331)pg/mL,亦明顯高于輕度OSAHS組患兒血清CysLTs水平(642.206±77.250)pg/mL,各組間有明顯差異(P均<0.05)。(4)當(dāng)患兒血清CysLTs水平為560.872pg/mL時(shí)為最佳臨界點(diǎn),,此臨界點(diǎn)的靈敏度(92.7%)和特異度(94.1%)均最高,誤診率和漏診率較小,診斷符合率為93.3%。結(jié)論:OSAHS組血清CysLTs水平高于對照組,CysLTs水平與OSAHS的嚴(yán)重程度密切相關(guān)。不同民族OSAHS患兒血清CysLTs水平有差異,血清CysLTs水平可能作為一個血清標(biāo)志物來初步篩查可疑OSAHS患兒。
[Abstract]:Objective: to investigate the changes and significance of serum cysteine leukotriene (CysLTs) levels in children with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods: 41 children with OSAHS were divided into three groups according to the level of AHI, mild, moderate and severe, and 34 in control group. All subjects were monitored by polysomnography. The level of serum CysLTs was determined by competitive enzyme linked immunosorbent assay (Elisa). Results the serum CysLTs level in the OSAHS group was 683.102 鹵89.825 渭 g / mL higher than that in the control group (461.985 鹵84.951g / mL). The difference was statistically significant (P < 0.05). 2. The serum CysLTs level and the apnea hypopnea index (AHI) in the OSAHS group were significantly higher than those in the control group (P < 0.05). There was a positive correlation between LHT and LHT (0.417 鹵0.422 鹵0.208, P < 0.05), and the lowest oxygen saturation (LSaO _ 2), and the lowest oxygen saturation (LSaO _ 2). Serum CysLTs level in severe OSAHS group was 773.118 鹵92.015 PG / mL higher than that in moderate OSAHS group (712.144 鹵41.331pgmL) and significantly higher than that in mild OSAHS group (642.206 鹵77.250 PG / mL). When the serum CysLTs level was 560.872pg / mL, it was the best critical point. The sensitivity and specificity of this critical point were the highest, and the misdiagnosis rate and missed diagnosis rate were small. Conclusion the level of serum CysLTs is significantly higher in the control group than that in the control group, and it is closely related to the severity of OSAHS. There are differences in serum CysLTs levels among the children with OSAHS from different nationalities. Serum CysLTs level may be used as a serum marker to screen suspected OSAHS children.
