【摘要】：特發(fā)性黃斑前膜的標(biāo)準(zhǔn)手術(shù)方案是玻璃體切割聯(lián)合前膜剝除術(shù)。然而同樣成功的手術(shù)后患者視力、視物變形等功能和黃斑區(qū)解剖結(jié)構(gòu)的預(yù)后卻不盡相同。通過查閱大量相關(guān)文獻(xiàn),發(fā)現(xiàn)許多影響術(shù)后解剖和功能恢復(fù)的預(yù)期影響因素。術(shù)前因素中除術(shù)前最佳矯正視力和病程等我們熟知的影響因素外,頻域光學(xué)相干斷層成像技術(shù)的應(yīng)用提供了更多黃斑前膜組織結(jié)構(gòu)的改變,如術(shù)前黃斑中心區(qū)厚度、內(nèi)核層厚度和光感受器細(xì)胞層連續(xù)性的改變等,這些信息可以作為我們推測(cè)預(yù)后的指標(biāo);手術(shù)方式、聯(lián)合用藥等術(shù)中因素和術(shù)后眼局部和全身用藥情況均影響患者術(shù)后解剖和功能預(yù)后。
[Abstract]:The standard procedure for idiopathic macular membrane surgery is vitrectomy combined with anterior membrane excision. However, the function of visual deformation and the prognosis of macular anatomic structure are not the same. In addition to the best preoperative corrected visual acuity and course of disease, we found many expected factors affecting postoperative anatomical and functional recovery, such as the best preoperative corrected visual acuity, the course of disease, and so on. The application of frequency-domain optical coherence tomography provides more changes in the tissue structure of the premacular membrane, such as the thickness of the central macular area, the thickness of the nuclear layer and the continuity of the photoreceptor cell layer, etc. These information can be used as a predictor of prognosis, surgical methods, combined use of drugs and other intraoperative factors, as well as postoperative local and systemic use of drugs all affect the postoperative anatomic and functional prognosis of patients.
