【摘要】：目的評價中藥液沖洗治療對慢性鼻竇炎患者鼻內(nèi)鏡術(shù)后術(shù)腔恢復的臨床療效和安全性。 方法通過計算機檢索電子數(shù)據(jù)庫和手工檢索重要期刊的方式,收集慢性鼻竇炎鼻內(nèi)鏡術(shù)后使用中藥液沖洗治療的隨機對照試驗(RCT)文獻,按納入及排除標準篩選文獻、評價研究質(zhì)量、提取文獻有效數(shù)據(jù),并采用RevMan 4.2.10進行統(tǒng)計分析。 結(jié)果3個亞組共納入32篇中文文獻,經(jīng)過以療效為主要終點指標和以術(shù)后癥狀改善評級、黏膜形態(tài)、鼻腔纖毛運動速率、術(shù)腔清潔時間、術(shù)腔上皮化時間、鼻腔免疫功能指標、不良反應(yīng)等次要結(jié)局指標的meta分析后,結(jié)果提示鼻內(nèi)鏡術(shù)后使用中藥液沖洗術(shù)腔的療效優(yōu)于單純生理鹽水或西藥液沖洗的可能性較高。安全性方面,由于本文中納入的關(guān)于不良反應(yīng)的文獻報道較少,在現(xiàn)有研究結(jié)果下尚不能證明中藥液沖洗術(shù)腔治療方法存在優(yōu)于單純生理鹽水或西藥液沖洗的可能。 結(jié)論在慢性鼻竇炎鼻內(nèi)鏡術(shù)后的治療中,采用中藥液沖洗治療可提高臨床療效、加快愈合時間,不良反應(yīng)發(fā)生率較小,但由于納入試驗方法學質(zhì)量普遍較低,評價指標合理的文獻較少,尚需進一步開展設(shè)計科學、指標合理的隨機對照試驗。
[Abstract]:Objective to evaluate the clinical efficacy and safety of Chinese medicine irrigation in patients with chronic sinusitis after endoscopic sinus surgery. Methods by means of computer searching electronic database and manual retrieval of important journals, the literature of RCTs of the patients with chronic sinusitis treated with traditional Chinese medicine after endoscopic surgery was collected, and the literature was screened according to the inclusion and exclusion criteria. The quality of the study was evaluated and the effective data were extracted and analyzed by RevMan 4.2.10. Results 32 articles of Chinese literature were included in the three subgroups. The main endpoints were curative effect and the improvement of postoperative symptoms, mucosal morphology, ciliary movement rate, cleaning time and epithelialization time. After meta analysis of the secondary outcome indexes such as nasal immune function and adverse reactions, the results showed that the efficacy of using Chinese medicine to wash the cavity after nasal endoscopy was better than that of saline or western medicine. Because there are few reports on adverse reactions in this paper, it is not possible to prove that the Chinese medicine solution washing method is superior to the normal saline or western medicine solution washing method under the existing research results. Conclusion in the treatment of chronic sinusitis after endoscopic sinus surgery, the use of Chinese medicine liquid washing therapy can improve the clinical efficacy, accelerate the healing time, and the incidence of adverse reactions is small, but the quality of the methodology is generally low. There are few literatures on reasonable evaluation index, so it is necessary to further develop the randomized controlled trial with scientific design and reasonable index.
