鼻中隔偏曲的鼻阻力相關(guān)因素探討

【摘要】:目的:探討一種能結(jié)合形態(tài)和功能對鼻中隔偏曲進行客觀評估的方法,為臨床上選擇恰當?shù)闹委煼绞教峁﹨⒖家罁?jù)。 方法:對符合條件的20例鼻中隔偏曲患者進行如下測量:1.利用多層螺旋CT攝片,在CT工作臺上直接測量研究對象的S總及S骨,并通過S軟=S總-S骨計算得出S軟(以S總表示下鼻甲前端橫截面積、S骨表示下鼻甲前端鼻骨的橫截面積、S軟表示下鼻甲前端軟組織的面積)。2.利用鼻阻力測量分析儀測量研究對象使用呋麻滴鼻液前后的鼻阻力。將橫截面積進行同側(cè)與對側(cè)比較、用藥前后鼻阻力差值進行同側(cè)與對側(cè)的比較。 結(jié)果:CT片上所測量橫截面積(cm2):同側(cè)S總:1.08±0.34;對側(cè)S總:1.35±0.55P;同側(cè)S骨:0.33±0.10;對側(cè)S骨:0.41±0.29;同側(cè)S軟:0.64±0.31;對側(cè)S軟:1.18±0.46 ;用呋麻滴鼻液前的鼻阻力(Pa/cm3/s):同側(cè)0.49±0.25;對側(cè):0.43±0.25;用呋麻滴鼻液后的鼻阻力(Pa/cm3/s):同側(cè)0.28±0.11;對側(cè):0.30±0.18。將同側(cè)S總與對側(cè)S總進行比較,發(fā)現(xiàn)同側(cè)下鼻甲前端橫截面積小于對側(cè),且其差異有統(tǒng)計學意義(P均0.05);將同側(cè)S骨與對側(cè)S骨進行比較,發(fā)現(xiàn)對側(cè)下鼻甲前端骨質(zhì)橫截面積大于同側(cè),但其差異無統(tǒng)計學意義(P0.05);將同側(cè)S軟與對側(cè)S軟進行比較,發(fā)現(xiàn)對側(cè)下鼻甲前端軟組織的橫截面積大于同側(cè),且其差異具有統(tǒng)計學意義(P0.05);將使用呋麻滴鼻液前測量的鼻阻力進行同側(cè)鼻阻力與對側(cè)鼻阻力的比較,發(fā)現(xiàn)同側(cè)鼻阻力略大于對側(cè),但這種差異無統(tǒng)計學意義(P0.05);將使用呋麻滴鼻液前后測量的鼻阻力差值進行同側(cè)鼻阻力與對側(cè)鼻阻力的比較,發(fā)現(xiàn)對側(cè)鼻阻力的變化較同側(cè)大,且其差異具有統(tǒng)計學意義(P0.05)。 結(jié)論: 1.鼻中隔偏曲患者的對側(cè)下鼻甲粘膜較同側(cè)肥大; 2.鼻中隔偏曲患者用藥前后對側(cè)鼻阻力變化大; 3.CT結(jié)合鼻阻力可作為臨床上對鼻中隔偏曲患者進行術(shù)前評估的一種客1觀、有效的方法。
[Abstract]:Objective: to explore an objective evaluation method of nasal septum deviation combined with morphology and function, and to provide reference for clinical treatment. Methods: twenty patients with nasal septum deviation were measured as follows: 1. The total S and S bone of the subjects were measured directly on the CT workbench using multislice spiral CT. And through S soft S total S bone calculation, S soft (S total denotes cross sectional area of anterior inferior turbinate and S bone denotes cross sectional area of anterior nasal bone of inferior turbinate; S soft represents area of soft tissue of anterior inferior turbinate. The nasal resistance of subjects before and after using furosemide nasal drops was measured by analyzer. The cross-sectional area was compared in the same side and opposite side. Nasal resistance difference before and after treatment was compared between ipsilateral and contralateral. Results the cross sectional area measured on the CT scan was as follows: ipsilateral S: 1.08 鹵0.34; contralateral S: 1.35 鹵0.55P; ipsilateral S bone: 0.33 鹵0.10; contralateral S bone: 0.41 鹵0.29; ipsilateral S soft: 0.64 鹵0.31; contralateral S: 1.18 鹵0.46; The nasal resistance before furosemide: ipsilateral 0.49 鹵0.25; contralateral 0.43 鹵0.25; nasal resistance: ipsilateral 0.28 鹵0.11; contralateral 0.30 鹵0.18. compare the total ipsilateral S with the contralateral S, and find that the cross sectional area of the ipsilateral inferior turbinate is less than that of the contralateral. The cross-sectional area of ipsilateral S bone and contralateral S bone was found to be larger than that of ipsilateral inferior turbinate, but the difference was not statistically significant (P 0.05), the ipsilateral S soft and contralateral S soft were compared. It was found that the cross sectional area of the anterior soft tissue of the contralateral inferior turbinate was larger than that of the ipsilateral, and the difference was statistically significant (P 0.05), and the nasal resistance measured before the use of furosemide nasal drops was compared between the ipsilateral nasal resistance and the contralateral nasal resistance. It was found that the ipsilateral nasal resistance was slightly greater than that of the contralateral side, but the difference was not statistically significant (P 0.05). The difference between the ipsilateral nasal resistance and the contralateral nasal resistance was compared before and after the use of furoma nasal drops, and the change of the contralateral nasal resistance was greater than that of the ipsilateral nasal resistance. The difference was statistically significant (P 0.05). Conclusion:. 1. The contralateral inferior turbinate mucosa in the patients with nasal septum deviation was higher than that in the ipsilateral side. 2.The contralateral nasal resistance of patients with nasal septum deviation changed greatly before and after treatment. 3. Ct combined with nasal resistance can be used as an effective method for preoperative evaluation of nasal septum deviation.
