【摘要】：目的:評(píng)估乳突切開(kāi)術(shù)并一期行聽(tīng)骨鏈重建的臨床療效。方法:對(duì)2008-01-2011-06期間在我科住院行乳突切開(kāi)術(shù)并一期鈦人工聽(tīng)骨植入手術(shù)并且有完整隨訪資料的患者139例進(jìn)行回顧分析,其中置入部分聽(tīng)骨贗復(fù)物(PORP)者91例,置入全聽(tīng)骨贗復(fù)物(TORP)者48例,隨訪時(shí)間為2~5年,純音測(cè)聽(tīng)法(PTA)檢測(cè)患者術(shù)前及術(shù)后聽(tīng)力,比較手術(shù)前后的氣導(dǎo)聽(tīng)閾(0.5、1.0、2.0、4.0kHz四個(gè)頻率氣導(dǎo)之平均值)及氣骨導(dǎo)差(ABG),分析鈦人工聽(tīng)骨在同期聽(tīng)骨鏈重建術(shù)的聽(tīng)力重建效果。術(shù)后ABG≤20dB為聽(tīng)力提高有效。結(jié)果:PORP植入患者術(shù)前氣導(dǎo)平均聽(tīng)閾為(53.97±11.32)dB,術(shù)后為(36.80±11.68)dB,平均降低(17.17±5.79)dB;術(shù)前ABG平均為(31.84±6.17)dB,術(shù)后為(15.13±7.22)dB,平均縮小(16.71±5.50)dB;TORP組患者術(shù)前氣導(dǎo)平均聽(tīng)閾為(58.05±11.35)dB,術(shù)后為(44.53±13.15)dB,平均降低(13.52±7.81)dB;術(shù)前ABG平均為(35.67±5.73)dB,術(shù)后為(21.48±7.01)dB,平均縮小(14.18±7.53)dB;各組術(shù)前術(shù)后的差異均有統(tǒng)計(jì)學(xué)意義(P0.01)。PORP組術(shù)后ABG≤20dB者(術(shù)后聽(tīng)力提高有效)共68例,有效率為74.73%;TORP組術(shù)后ABG≤20dB者共26例,有效率為54.13%;總有效率為68.63%,兩組之間的差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:乳突切開(kāi)術(shù)并同期鈦人工聽(tīng)骨植入取得了良好的聽(tīng)力效果,PORP比TORP的聽(tīng)力改善效果好。
[Abstract]:Objective: to evaluate the clinical effect of mastoidectomy and ossicular chain reconstruction. A retrospective analysis was made of 139 patients who underwent mastoidectomy and one-stage titanium ossicular implantation in our department during the period from 2008-01-2011-06 and had complete follow-up data.
