【摘要】： 目的:評(píng)價(jià)重組人P53腺病毒注射液(rAd-p53)聯(lián)合放療治療鼻咽癌的療效及可能出現(xiàn)的毒副作用。 方法:選擇安徽醫(yī)科大學(xué)第一附屬醫(yī)院病理科診斷證實(shí)的III、IV期原發(fā)鼻咽癌16例,兩組性別、年齡、病灶大小、照射野、放療劑量均衡性檢驗(yàn)均無統(tǒng)計(jì)學(xué)意義(P0.05)。鼻咽癌隨機(jī)分為基因治療聯(lián)合放療組(治療組)8例和單純放療組(對(duì)照組)8例,治療組:每周五予重組人P53腺病毒注射液1 ml鼻咽部瘤體注射,每周一開始行面頸聯(lián)合野放療,每日2 Gy,連續(xù)5 d,連續(xù)治療6周。重組人P53腺病毒注射液由深圳賽百諾基因技術(shù)有限公司生產(chǎn),規(guī)格:1×1012 VP/支。使用藥物時(shí)從-20℃冰箱取出,室溫靜置融化后半小時(shí)內(nèi)使用。采用鼻竇內(nèi)窺鏡引導(dǎo)下瘤內(nèi)注射給藥方式,腫瘤φ4 cm的,用生理鹽水稀釋到2 ml使用;腫瘤φ4 cm用生理鹽水稀釋到4 ml后使用。每次瘤體注射藥物時(shí)均拍攝照片,以便自身前后對(duì)照。對(duì)照組:每周連續(xù)5 d行鼻咽癌面頸聯(lián)合野常規(guī)放療,每日2 Gy,連續(xù)治療6周。治療前3天、治療后1周及治療后1個(gè)月兩組分別予血常規(guī)、血生化、尿常規(guī)和心電圖檢查;治療前3天及治療后1個(gè)月分別予鼻咽部CT檢查。兩組鼻咽部劑量均為70 Gy/35次,頸部淋巴結(jié)放療劑量均為60Gy/30次。 結(jié)果: 1.兩組均予鼻咽部CT掃描檢查,對(duì)照組治療后一月與治療前相比,瘤體退縮不明顯。 2.治療組治療后一個(gè)月與治療前相比,瘤體退縮明顯。 3.自身對(duì)照比較,治療前后瘤體退縮明顯,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。 4.治療組治療前后血常規(guī)、尿常規(guī)、肝腎功能相關(guān)指標(biāo)差異無統(tǒng)計(jì)學(xué)意義,心電圖未見明顯異常。 5.除了自限性低熱外,未見其它不良反應(yīng)。 6.基本狀況各項(xiàng)評(píng)分中,除情感狀況評(píng)分差異有統(tǒng)計(jì)學(xué)意義外,其余幾項(xiàng)評(píng)分均無統(tǒng)計(jì)學(xué)意義。 結(jié)論: 1.重組人P53腺病毒注射液聯(lián)合放療治療鼻咽癌具有較好療效。 2.重組人P53腺病毒注射液聯(lián)合放療治療鼻咽癌安全,不良反應(yīng)患者可以耐受。
[Abstract]:Aim: to evaluate the efficacy and side effects of recombinant human adenovirus p53 (rAd-p53) combined with radiotherapy in the treatment of nasopharyngeal carcinoma (NPC). Methods: sixteen cases of primary stage IIIIV nasopharyngeal carcinoma confirmed by pathology in the first affiliated Hospital of Anhui Medical University were selected. The two groups were sex, age, lesion size and irradiation field. There was no statistical significance in dose balance test. Nasopharyngeal carcinoma was randomly divided into gene therapy combined with radiotherapy group (8 cases) and radiotherapy group (control group 8 cases). Treatment group: 1 ml of recombinant human p53 adenovirus injection was injected into nasopharynx tumor every Friday. The patients were treated with face neck combined with field radiotherapy at the beginning of one week, 2 Gy daily for 5 days. The recombinant human p53 adenovirus injection was produced by Shenzhen Sebenor Gene Technology Co., Ltd., with the specification of 1 脳 1012 VP1 / branch.
