【摘要】：目的觀察改良瞼板腺壓榨法治療瞼板腺功能障礙的效果。方法選擇瞼板腺功能障礙患者74例,其中32例(對(duì)照組)采用傳統(tǒng)瞼板腺壓榨法治療,42例(觀察組)采用改良瞼板腺壓榨法治療,隨訪2個(gè)月。治療前后,行眼表面疾病指數(shù)(OSDI)、瞼板腺積分評(píng)價(jià)及淚液分泌試驗(yàn)(SIT),采用濾紙條檢查淚膜破碎時(shí)間(BUT),結(jié)膜熒光素鈉染色檢查(FL)評(píng)價(jià)結(jié)膜病變;采用印跡細(xì)胞法于壓榨處的下眼瞼結(jié)膜面取材,PAS染色后觀察壓榨處結(jié)膜損傷情況;記錄手術(shù)時(shí)間及患者術(shù)后不適感。結(jié)果兩組治療后,BUT、SIT較治療前提高,OSDI、瞼板腺積分及FL下降,差異有統(tǒng)計(jì)學(xué)意義(P均0.05);治療后,觀察組BUT、SIT高于對(duì)照組,OSDI、瞼板腺積分及FL低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P均0.05)。觀察組瞼結(jié)膜上皮損傷1級(jí)30例、2級(jí)12例,對(duì)照組瞼結(jié)膜上皮損傷3級(jí)13例、4級(jí)19例,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.01)。觀察組手術(shù)時(shí)間(2.6±0.7)min,短于對(duì)照組的(14.9±3.3)min(P0.01)。觀察組20例訴有眼部刺激感,其中9例為明顯刺激感、11例為輕度刺激感;對(duì)照組32例術(shù)后均訴眼部有刺激感,其中25例主訴有明顯刺激感、7例訴有輕度刺激感;兩組術(shù)后不適情況比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論改良瞼板腺壓榨法治療瞼板腺功能障礙臨床療效好、損傷小。
[Abstract]:Objective to observe the effect of modified tarsal gland pressing on tarsal gland dysfunction. Methods 74 patients with tarsal gland dysfunction were selected. 32 patients (control group) were treated with traditional tarsal gland pressing method and 42 patients (observation group) were treated with modified tarsal gland pressing method. The patients were followed up for 2 months. Before and after treatment, the eye surface disease index (OSDI),) tarsal gland score and tear secretion test (SIT), were used to evaluate the conjunctival lesions by filter paper strip, (BUT), conjunctival fluorescein sodium staining was used to evaluate conjunctival lesions, and imprinted cell method was used to take materials from the conjunctival surface of the lower eyelid at the pressing site, and the conjunctival injury at the pressing site was observed after PAS staining. The operation time and postoperative discomfort of the patients were recorded. Results after treatment, BUT,SIT increased, OSDI, tarsal gland score and FL decreased significantly in the two groups, BUT,SIT in the observation group was higher than that in the control group, OSDI, tarsal gland score and FL were significantly lower than those in the control group (P < 0.05). There were 30 cases of grade 1 conjunctival epithelial injury and 12 cases of grade 2 eyelid conjunctival epithelial injury in the observation group, and 13 cases in the control group were grade 3 and 19 cases in grade 4. There was significant difference between the two groups (P 0.01). The operation time of the observation group (2.6 鹵0.7) min, was shorter than that of the control group (14.9 鹵3.3) min (P 0.01). In the observation group, 20 cases complained of ocular irritation, including 9 cases of obvious irritation and 11 cases of mild irritation, while 32 cases of the control group complained of irritation of the eye after operation, of which 25 cases complained of obvious irritation and 7 cases complained of mild irritation. There was significant difference between the two groups (P 0.05). Conclusion the modified tarsal gland pressing method is effective in the treatment of tarsal gland dysfunction.
