【摘要】：目的:比較玻璃體腔內(nèi)注射雷珠單抗在不同類型的息肉狀脈絡(luò)膜血管病變治療中的臨床療效。方法:選取2013-05/2015-05在紹興市人民醫(yī)院治療的63例63眼息肉狀脈絡(luò)膜血管病變患者作為研究對象,根據(jù)息肉數(shù)的差異分為單息肉組(38例38眼)和多息肉組(25例25眼),兩組患者均接受玻璃體腔內(nèi)注射雷珠單抗治療,每月1次,連續(xù)注射3mo。注射后隨訪1a,比較兩組患者治療后最佳矯正視力(best corrected visual acuity,BCVA)和中心視網(wǎng)膜厚度(central retinal thickness,CRT)的變化。結(jié)果:與多息肉組比較,單息肉組治療前最大線性直徑更大、纖維血管性視網(wǎng)膜色素上皮脫離發(fā)生率更低、BCVA更低(P0.05)。治療后3、6mo,1a單息肉組BCVA明顯低于多息肉組(P0.05);單息肉組治療后3、6mo,1a時BCVA較治療前均顯著下降(P0.05)。單息肉組和多息肉組患者的CRT在治療后6、12mo有統(tǒng)計(jì)學(xué)差異(P0.05);單息肉組治療后3、6mo,1a時CRT均較治療前低(P0.05),多息肉組治療后3、6mo的CRT較治療前低(P0.05)。結(jié)論:玻璃體腔內(nèi)注射雷珠單抗在多息肉型息肉狀脈絡(luò)膜血管病變中治療效果更好,息肉數(shù)量對于評估預(yù)后具有重要意義。
[Abstract]:Objective: to compare the clinical efficacy of intravitreal injection of Leizhu monoclonal antibody in the treatment of different types of polypoid choroid vascular diseases. Methods: 63 patients (63 eyes) with polypoid choroid angiopathy treated in Shaoxing people's Hospital were divided into single polyposis group (38 eyes, 38 eyes) and polypolyposis group (25 eyes, 25 eyes) according to the difference of polyposis number. After injection, the changes of best corrected visual acuity (best corrected visual acuity,BCVA) and central retinal thickness (central retinal thickness,CRT) were compared between the two groups. Results: compared with polyposis group, the maximum linear diameter of single polyposis group was larger, the incidence of fibrovascular retinal pigment epithelial detachment was lower, and BCVA was lower than that of polyposis group before treatment (P 0.05). At 3, 6 months after treatment, BCVA in 1a group was significantly lower than that in polyposis group (P 0.05), and BCVA in single polyposis group was significantly lower than that before treatment at 3, 6 months after treatment (P 0.05). The CRT of patients in single polyposis group and polyposis group was significantly different at 6 months and 12 months after treatment (P 0.05), and the CRT in single polyposis group was lower than that before treatment at 3 and 6 months after treatment (P 0.05), and the CRT at 6 months after treatment in polyposis group was lower than that before treatment (P 0.05). Conclusion: intravitreal injection of Leizhu monoclonal antibody is more effective in the treatment of polypolyposis polyposis choroid vascular lesions, and the number of polyps is of great significance in evaluating the prognosis.
