【摘要】：目的:觀察助聽器對(duì)耳聾伴有耳鳴患者的耳鳴治療效果。方法:收集2014年10月至2016年10月間在我院就診的耳聾伴有耳鳴的患者資料,將接受佩戴助聽器治療的、隨訪資料完整的45例雙耳中度以上耳聾且耳鳴病程大于3個(gè)月的患者納入該研究,隨訪1年,觀察在治療3、6、12個(gè)月后,助聽器對(duì)耳鳴治療總有效率并且分別觀察在治療3、6、12個(gè)月后,助聽器對(duì)不同程度耳聾、耳聾和耳鳴發(fā)生先后順序、不同性質(zhì)耳鳴、不同音調(diào)耳鳴、不同耳鳴病程的耳鳴有效率,并使用SPSS20.0軟件包進(jìn)行統(tǒng)計(jì)分析。結(jié)果:1.助聽器治療3、6、12個(gè)月后療效比較治療12個(gè)月有效率最高,6個(gè)月次之,3個(gè)月最低,其中治療3個(gè)月和治療6個(gè)月的有效率差異有顯著性(P0.05),但治療6個(gè)月和治療12個(gè)月的有效率差異無顯著性(P0.05)。2.助聽器對(duì)不同耳聾程度組的療效比較中度耳聾組有效率最高、中重度組次之,重度耳聾組最低,但三組間比較無顯著差異(P0.05),只有中度耳聾組與重度耳聾組治療12個(gè)月有顯著差異(P0.05)。3.助聽器對(duì)耳鳴和耳聾發(fā)生先后順序組的療效比較耳鳴和耳聾同時(shí)發(fā)生組有效率最高,耳鳴后于耳聾發(fā)生組次之,耳鳴先于耳聾發(fā)組最低,但三組間比較均無顯著差異(P0.05),只有耳鳴先于耳聾組和耳鳴和耳聾同時(shí)發(fā)生組在治療12個(gè)月有顯著差異(P0.05)。4.助聽器對(duì)不同耳鳴性質(zhì)組的療效比較單聲耳鳴改善的有效率最高,雙聲組次之、復(fù)合聲組最低,但三組之間無顯著差異(P0.05)。5.助聽器對(duì)不同耳鳴音調(diào)組的療效比較耳鳴低調(diào)組有效率稍高于高調(diào)組,兩組組間無顯著差異(P0.05)。6.助聽器對(duì)不同耳鳴病程組的療效比較耳鳴病程小于1年組耳鳴改善的有效率高于耳鳴病程大于1年組;僅在治療12個(gè)月后,兩組有顯著差異(P0.05),其余組間無顯著差異(P0.05)。結(jié)論:1.助聽器對(duì)伴中度以上耳聾患者的耳鳴在治療3~6個(gè)月后有明顯效果;2.助聽器對(duì)中度耳聾、耳鳴和耳聾同時(shí)發(fā)生、單聲耳鳴、低調(diào)耳鳴的耳聾患者的耳鳴治療效果較好;3.伴有中度以上耳聾的耳鳴患者應(yīng)及時(shí)、盡早佩戴助聽器。
[Abstract]:Objective: to observe the effect of hearing aid on tinnitus in patients with deafness and tinnitus. Methods: the data of deafness patients with tinnitus who were treated in our hospital from October 2014 to October 2016 were collected. 45 patients with moderate deafness in both ears and with a course of tinnitus more than 3 months were followed up for 1 year. The total effective rate of hearing aid in the treatment of tinnitus was observed after 3, 6 and 12 months of treatment, and the total effective rate of hearing aid in the treatment of tinnitus was observed after 3, 6 and 12 months of treatment, respectively. Hearing aids were used to analyze the effective rate of tinnitus in different degrees of deafness, deafness and tinnitus, tinnitus of different nature, tinnitus of different tones and course of tinnitus. SPSS20.0 software package was used to analyze the effective rate of tinnitus in different degrees of deafness, deafness and tinnitus. Result: 1. After 12 months of hearing aid treatment, the effective rate was the highest in 12 months, the second in 6 months, and the lowest in 3 months. There was significant difference in the effective rate between 3 months and 6 months (P 0.05), but there was no significant difference between 6 months and 12 months (P 0.05). Compared with the moderate deafness group, the hearing aid group had the highest effective rate, the moderate and severe deafness group was the second, and the severe deafness group was the lowest, but there was no significant difference among the three groups (P 0.05). There was only significant difference between the moderate deafness group and the severe deafness group for 12 months (P 0.05). The curative effect of hearing aid on tinnitus and deafness was the highest in the tinnitus and deafness group, the second in the deafness group after tinnitus, and the lowest in the deafness group before the deafness group, but there was no significant difference among the three groups (P 0.05). There was significant difference in tinnitus before deafness group and tinnitus and deafness group at 12 months (P 0.05). Compared with the different tinnitus nature groups, the effective rate of monophonic tinnitus was the highest, that of the double sound group was the second, and that of the compound sound group was the lowest, but there was no significant difference among the three groups (P 0.05). Comparison of the curative effect of hearing aids on different tinnitus tone groups the effective rate of tinnitus low-key group was slightly higher than that of high-profile group, and there was no significant difference between the two groups (P 0.05). 6. The effective rate of tinnitus improvement in tinnitus course less than 1 year group was higher than that in tinnitus course group more than 1 year group; only 12 months after treatment, there was significant difference between the two groups (P 0.05), but there was no significant difference between the other groups (P 0.05). Conclusion: 1. Hearing aids had obvious effect on tinnitus in patients with moderate deafness after 3 to 6 months of treatment. Hearing aids are effective in the treatment of deafness in patients with moderate deafness, tinnitus and deafness at the same time, monophonic tinnitus and low-key tinnitus. Tinnitus patients with moderate deafness should wear hearing aids as soon as possible.
