【摘要】：目的對(duì)鼻咽癌患者唾液、血清及外周血白細(xì)胞中的EBV-DNA進(jìn)行檢測(cè)，分析唾液、血清、外周血白細(xì)胞中的EB病毒DNA含量與鼻咽癌的相關(guān)性。 方法對(duì)40例初診鼻咽癌患者和50例正常人的唾液、血清及外周血白細(xì)胞中的EBV-DNA進(jìn)行熒光聚合酶鏈反應(yīng)(FQ-PCR)檢測(cè)。 結(jié)果EBV-DNA在鼻咽癌患者血清中的檢出率為67.5%，顯著高于正常人血清中的EBV-DNA檢出率2%(2=44.48，，P0.001)；鼻咽癌患者的唾液、外周血白細(xì)胞EBV-DNA檢出率與正常人的無(wú)顯著差異性；鼻咽癌患者和正常人的唾液EBV-DNA拷貝數(shù)與外周血白細(xì)胞EBV-DNA拷貝數(shù)呈正相關(guān)性；鼻咽癌患者血清EBV-DNA拷貝數(shù)與臨床TNM分期有關(guān)，分期越晚含量越高。 結(jié)論研究結(jié)果提示EB病毒長(zhǎng)期存在于多數(shù)人的唾液中，也可存在于外周血B淋巴細(xì)胞中，并不能認(rèn)為是一種癌前狀態(tài)；EB病毒感染發(fā)生異型改變的鼻咽上皮可視為癌前狀態(tài)。血清EBV-DNA檢測(cè)在鼻咽癌的臨床診斷中具有較高的敏感性（67.5%）和特異性（98%），有助于鼻咽癌的早期診斷；血清EBV-DNA拷貝數(shù)與臨床TNM分期有關(guān)，可在分子水平對(duì)鼻咽癌的TNM分期進(jìn)行補(bǔ)充。
[Abstract]:Aim to detect EBV-DNA in saliva, serum and peripheral blood leukocytes in patients with nasopharyngeal carcinoma (NPC), and analyze the correlation between the DNA content of EB virus in saliva, serum and peripheral blood leukocytes and nasopharyngeal carcinoma (NPC). Methods fluorescent polymerase chain reaction (FQ-PCR) was used to detect EBV-DNA in saliva, serum and peripheral blood leukocytes of 40 newly diagnosed NPC patients and 50 normal controls. Results the positive rate of EBV-DNA in serum of NPC patients was 67.5%, which was significantly higher than that of normal subjects (2%, 44.48, P0.001). There was no significant difference between the detection rate of EBV-DNA in saliva and peripheral blood leukocytes of NPC patients and normal persons, but there was a positive correlation between the EBV-DNA copy number of saliva and the EBV-DNA copy number of peripheral blood leukocytes in NPC patients and normal persons. The copy number of serum EBV-DNA in NPC patients was related to clinical TNM staging, and the later the staging, the higher the content. Conclusion the results suggest that EB virus exists in saliva and B lymphocytes of peripheral blood for a long time and can not be regarded as a precancerous state, and the nasopharynx epithelium infected with EB virus may be regarded as precancerous state. The detection of serum EBV-DNA has a high sensitivity (67.5%) and specificity (98%) in the clinical diagnosis of nasopharyngeal carcinoma, which is helpful for the early diagnosis of nasopharyngeal carcinoma. The serum EBV-DNA copy number was correlated with clinical TNM staging, and it could be used to supplement the TNM staging of nasopharyngeal carcinoma at molecular level.
