【摘要】：目的探討微創(chuàng)鎢針刀電凝模式行扁桃體部分切除治療兒童伴扁桃體肥大的阻塞性睡眠呼吸暫停低通氣綜合征(OSAHS)的臨床療效。方法回顧分析收治的60例OSAHS伴扁桃體肥大患兒,觀察組采用微創(chuàng)鎢針刀電凝模式行包膜內(nèi)扁桃體部分切除術(shù);對照組采用傳統(tǒng)剝離法切除扁桃體。對比分析兩組手術(shù)時(shí)間、術(shù)中出血量、術(shù)后次日疼痛指數(shù)及術(shù)前術(shù)后睡眠監(jiān)測結(jié)果。結(jié)果觀察組較對照組手術(shù)時(shí)間短、術(shù)后出血量少、術(shù)后疼痛輕,差異有統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)后3個(gè)月PSG監(jiān)測顯示兩組癥狀明顯緩解,組間差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論微創(chuàng)鎢針刀電凝模式行扁桃體部分切除術(shù)治療兒童伴扁桃體肥大的OSAHS優(yōu)于傳統(tǒng)扁桃體剝離法,值得臨床推廣。
[Abstract]:Objective to investigate the clinical efficacy of partial tonsillectomy with minimally invasive tungsten needle knife electrocoagulation in the treatment of obstructive sleep apnea hypopnea syndrome (OSAHS) in children with tonsillar hypertrophy. Methods 60 cases of OSAHS with tonsillar hypertrophy were analyzed retrospectively. The patients in the observation group were treated with partial tonsillectomy with tungsten needle knife electrocoagulation and the patients in the control group were excised with the traditional peeling method. The operative time, intraoperative bleeding volume, postoperative pain index and preoperative and postoperative sleep monitoring results were compared between the two groups. Results compared with the control group, the observation group had shorter operation time, less postoperative bleeding and less postoperative pain (P0.05), and the PSG monitoring at 3 months after operation showed that the symptoms of the two groups were obviously relieved, and there was no significant difference between the two groups (P0.05). Conclusion partial tonsillectomy in children with tonsillectomy with minimally invasive tungsten needle knife electrocoagulation is superior to traditional tonsillectomy in the treatment of OSAHS. It is worth popularizing in clinical practice.
