【摘要】：目的研究特發(fā)性黃斑裂孔(idiopathic macular holes,IMH)患者術(shù)前光學(xué)相干斷層掃描技術(shù)(optical coherence tomography,OCT)測(cè)量的相關(guān)參數(shù)與術(shù)后早期視力恢復(fù)的相關(guān)性。方法收集24例(25眼)IMH患者,均行玻璃體切割+內(nèi)界膜剝離+玻璃體內(nèi)注氣術(shù),術(shù)前、術(shù)后12周均行最佳矯正視力(best corrected visual acuity,BCVA)檢查,術(shù)前1周內(nèi)OCT影像上直接測(cè)量裂孔最大高度,通過Image-Pro Plus 6.0軟件測(cè)量黃斑裂孔最小徑、基底徑及外界膜光感受器斷裂點(diǎn)與視網(wǎng)膜色素上皮層的最大垂直高度,并計(jì)算黃斑孔指數(shù)(macular hole index,MHI)、牽拉孔指數(shù)(tractional hole index,THI)、黃斑孔指數(shù)'(macular hole index',MHI')及牽拉孔指數(shù)'(tractional hole index',THI'),應(yīng)用SPSS 22.0軟件包進(jìn)行Spearman相關(guān)性分析,得出各參數(shù)與術(shù)后12周時(shí)BCVA的相關(guān)性,受試者工作特征曲線擬合分析得出MHI、THI、MHI'、THI'界值。結(jié)果IMH患者術(shù)后12周BCVA較術(shù)前明顯提高且差異有統(tǒng)計(jì)學(xué)意義(P=0.000)。相關(guān)性分析結(jié)果表明:術(shù)后12周BCVA與術(shù)前MHI、THI、MHI'、THI'具有相關(guān)性(均為P0.05)。MHI≥0.705組術(shù)后12周時(shí)BCVA平均為0.297,好于MHI0.705組的0.103(P=0.023);THI≥1.425組術(shù)后12周時(shí)BCVA平均為0.288,好于THI1.425組的0.163(P=0.001)。結(jié)論THI≥1.425或MHI≥0.705者術(shù)后12周時(shí)能獲得較好的BCVA,可作為預(yù)測(cè)IMH患者術(shù)后早期視力恢復(fù)情況的指標(biāo)。
[Abstract]:Objective to study the correlation between preoperative optical coherence tomography (optical coherence tomography,OCT) and early postoperative visual acuity in patients with idiopathic macular hole (idiopathic macular holes,IMH). Methods Twenty-four patients (25 eyes) with IMH underwent intravitreal gas injection by vitrectomy with internal boundary membrane dissection. The best corrected visual acuity (best corrected visual acuity,BCVA) was performed before and 12 weeks after operation. The maximum height of the hiatus was measured directly on OCT images within 1 week before operation. The minimum diameter of macular hole, the basal diameter and the maximum vertical height of the outer membrane photoreceptor and the retinal pigment epithelium were measured by Image-Pro Plus 6.0 software. The macular pore index (macular hole index,MHI), the drag hole index (tractional hole index,THI), the macular pore index'(macular hole index',MHI') and the pull hole index'(tractional hole index',THI') were calculated. The correlation analysis of Spearman with SPSS 22.0 software package was carried out, and the correlation between each parameter and BCVA at 12 weeks after operation was obtained. The threshold value of MHI,THI,MHI',THI' was obtained by fitting the operating characteristic curve of the subjects. Results BCVA was significantly higher in IMH patients 12 weeks after operation than that before operation (P0. 000).
