【摘要】：目的:探討OSAHS伴胰島素抵抗(IR)患者術(shù)后脂肪因子水平的改變與臨床意義。方法:連續(xù)性納入2015-01-2016-08期間就診的84例OSAHS患者,根據(jù)IR情況分為IR組(52例)和非IR組(32例)。所有患者接受懸雍垂腭咽成形術(shù)(UPPP),并分別于術(shù)前、術(shù)后1周和3個(gè)月檢測(cè)chemerin、visfatin和obestatin 3種脂肪因子的水平。結(jié)果:IR組患者術(shù)前chemerin、visfatin和穩(wěn)態(tài)模型胰島素抵抗指數(shù)(HOMA-IR)水平明顯高于非IR組(P0.05),而obestatin水平明顯低于非IR組(P0.05)。術(shù)后1周時(shí)IR組obestatin水平明顯低于非IR組(P0.05),HOMA-IR明顯高于非IR組(P0.05)。組內(nèi)比較可見術(shù)后1周IR組患者chemerin、visfatin和HOMA-IR明顯下降(P0.05),obestitin明顯上升(P0.05)。UPPP術(shù)與△chemerin(r=-0.685,P0.05)和△HOMA-IR(r=-0.622,P0.05)呈負(fù)相關(guān),與△obestitin呈正相關(guān)(r=0.612,P0.05)�！鱄OMA-IR與△chemerin呈正相關(guān)(r=0.634,P0.05)。結(jié)論:OSAHS患者經(jīng)過(guò)手術(shù)治療后,chemerin、visfatin和obestatin的水平和IR均明顯改善,其中chemerin與IR改善情況最為相關(guān)。
[Abstract]:Objective: to investigate the changes and clinical significance of fat factor levels in OSAHS patients with insulin resistance (IR). Methods: 84 OSAHS patients who were admitted continuously from 2015-01-2016-08 were divided into IR group (n = 52) and non-IR group (n = 32) according to IR. All patients were treated with uvulopalatopharyngoplasty (UPPP), and the levels of chemininvisfatin and obestatin were measured before, 1 week and 3 months after operation. Results the levels of preoperative chemerininvisfatin and homeostasis model insulin resistance index (HOMA-IR) were significantly higher in the weight IR group than in the non-IR group (P0.05), but the level of obestatin was significantly lower than that in the non-IR group (P0.05). The level of obestatin in IR group was significantly lower than that in non-IR group (P0.05) and HOMA-IR was significantly higher than that in non-IR group at 1 week after operation (P0.05). One week after operation, the levels of chemerinine visfatin and HOMA-IR in IR group were significantly decreased (P0.05). (P0.05). There was a negative correlation between chemerin (r-0.685) and HOMA-IR (r-0.622), and a positive correlation between HOMA-IR and chemerin (rr 0.634 P 0.05). Conclusion the levels of visfatin and obestatin and IR in patients with OSAHS were significantly improved after surgical treatment, and the improvement of chemerin and IR was the most significant.
