【摘要】：目的:探討上頜竇后鼻孔息肉的起源及療效。方法:對(duì)57例上頜竇后鼻孔息肉患者進(jìn)行術(shù)前鼻內(nèi)鏡檢查、冠狀位和軸位CT掃描;分別采用鼻內(nèi)鏡下中鼻道上頜竇開窗術(shù)、聯(lián)合中鼻道及下鼻道開窗術(shù)、聯(lián)合鼻內(nèi)鏡下鼻腔側(cè)壁切開入路及中鼻道開窗3種手術(shù)方式;術(shù)中鼻內(nèi)鏡下觀察后鼻孔息肉與上頜竇內(nèi)病變的關(guān)系、組織病理學(xué)檢測(cè)結(jié)果;術(shù)后進(jìn)行隨訪。結(jié)果:57例上頜竇后鼻孔息肉起源于同側(cè)上頜竇囊腫,22例中鼻道上頜竇開窗術(shù),復(fù)發(fā)2例;17例聯(lián)合中鼻道及下鼻道開窗術(shù),復(fù)發(fā)1例;18例聯(lián)合鼻內(nèi)鏡下鼻腔側(cè)壁切開入路及中鼻道開窗術(shù),治愈率100%。結(jié)論:上頜竇后鼻孔息肉起源于上頜竇內(nèi)囊腫,囊壁由竇口脫出于中鼻道及鼻咽部形成息肉,常見于單側(cè),偶發(fā)雙側(cè)。鼻內(nèi)鏡下中鼻道上頜竇開窗術(shù)、聯(lián)合中鼻道及下鼻道開窗術(shù)有很好的療效,對(duì)于上頜竇前壁的囊腫聯(lián)合鼻內(nèi)鏡下鼻腔側(cè)壁切開入路可獲得較好的視野,有利于病變的完整切除。
[Abstract]:Objective: to investigate the origin and curative effect of posterior nostril polyp of maxillary sinus. Methods: Fifty-seven patients with nasal polyps of posterior maxillary sinus underwent preoperative nasal endoscopy, coronal and axial CT scanning, middle maxillary sinus fenestration under nasal endoscope, combined with middle nasal canal and inferior nasal canal fenestration, respectively. The relationship between posterior nostril polyp and maxillary sinus lesions was observed under nasal endoscope and the results of histopathological examination were followed up. Results 57 cases of posterior nostril polyps of maxillary sinus originated from ipsilateral maxillary sinus cyst 22 cases of middle nasal canal maxillary sinus fenestration, 2 cases of recurrence of 17 cases combined with middle nasal canal and inferior nasal canal fenestration. There were 18 cases of recurrence combined with endoscopic incision of the lateral wall of nasal cavity and fenestration of middle nasal canal. The cure rate was 100%. Conclusion: the posterior nasal polyp of maxillary sinus originated from the cyst in the maxillary sinus, and the cyst wall protruded from the sinus orifice to the middle nasal canal and nasopharynx to form polyps. Endoscopic fenestration of maxillary sinus, combined with middle nasal canal and inferior nasal canal fenestration has good curative effect. For the cyst of anterior wall of maxillary sinus combined with lateral wall incision of nasal cavity under nasal endoscope, a better visual field can be obtained, which is beneficial to the complete excision of the lesion.
