【摘要】：目的:探討額部軸型皮瓣修復(fù)外鼻皮膚組織缺損的臨床療效。方法:收集19例采用額部軸型皮瓣修復(fù)外鼻皮膚組織缺損的患者的臨床資料,其中外傷7例,惡性腫瘤10例,乳頭狀瘤1例,黑色素痣1例。4例行鼻背部修復(fù),5例行鼻側(cè)壁修復(fù),3例行鼻尖部修復(fù),7例行鼻翼再造及皮瓣整復(fù)術(shù)。所有病例轉(zhuǎn)移皮瓣蒂部均縫合為管狀,供皮區(qū)采用腹部全層游離皮片修復(fù)18例,減張后直接拉攏縫合1例。術(shù)后均行二期斷蒂。結(jié)果:19例患者轉(zhuǎn)移皮瓣全部存活,轉(zhuǎn)移皮瓣全覆蓋創(chuàng)面;再造鼻翼與顏面部皮膚、色澤接近;本組患者感覺測定分級為S3~S4,兩點辨別覺(2PD)3~6 mm;術(shù)后隨訪轉(zhuǎn)移皮瓣外形滿意,腫瘤病例無復(fù)發(fā)。結(jié)論:采用含知名血管的額部軸形皮瓣修復(fù)外鼻創(chuàng)面,可延長修復(fù)距離、擴大修復(fù)范圍,且額部皮瓣相對較厚,韌性強,利于鼻部塑形,操作簡便。
[Abstract]:Objective: to investigate the clinical effect of frontal axial flap for repairing skin defect of external nose. Methods: the clinical data of 19 patients with external nasal skin defects were collected, including 7 cases of trauma, 10 cases of malignant tumor and 1 case of papilloma. In 1 case of nevus, 4 cases were treated with nasal dorsal repair, 5 cases with nasal lateral wall repair, 3 cases with nasal tip repair and 7 cases with alar reconstruction and flap restoration. The pedicle of the flap was sutured to be tubular in all cases, 18 cases were repaired with free skin grafts of the whole abdomen, and 1 case was directly closed and sutured after tensioning. All patients underwent secondary pedicle amputation. Results all of the 19 patients survived and the flap covered the wound completely. The skin of the reconstructed nasal wing was similar to the skin of the face. The sensory grading of the patients was S3S4, 2PDD 3mm and 6mm, and the appearance of the flap was satisfactory after operation, and the skin of the reconstructed nasal wing was similar to that of the facial region, and the skin color was similar to that of the skin of the face. Tumor cases did not recur. Conclusion: using the frontal axial flap with well-known blood vessels to repair the external nasal wound can prolong the repair distance and expand the repair range. Moreover, the frontal flap is relatively thick, strong toughness, good for the shape of nose, and easy to operate.
