鼻咽癌DNA倍體性及CD133蛋白表達(dá)與放療近期療效的關(guān)系

【摘要】:目的: 檢測(cè)鼻咽癌組織的DNA倍體性及CD133蛋白表達(dá)情況,并探討二者與鼻咽癌放射敏感性及放射治療近期療效的關(guān)系。 方法: 分別采用流式細(xì)胞術(shù)和免疫組織化學(xué)染色法檢測(cè)56例初治鼻咽癌患者治療前腫瘤活檢標(biāo)本的DNA倍體性及CD133蛋白表達(dá)情況。另取10例鼻咽良性病變組織標(biāo)本作為對(duì)照。所有患者均采用常規(guī)方案放療,放療劑量達(dá)40Gy時(shí)按CT所示腫瘤退縮程度評(píng)價(jià)放射敏感性,放療結(jié)束時(shí)按RECIST實(shí)體瘤療效評(píng)價(jià)標(biāo)準(zhǔn)評(píng)價(jià)近期療效。結(jié)合臨床資料對(duì)56例鼻咽癌組織的DNA倍體性及CD133蛋白表達(dá)情況進(jìn)行分析,并探討其與鼻咽癌放射敏感性及放療近期療效的關(guān)系。 結(jié)果: 1、DNA異倍體及CD133蛋白在鼻咽癌中的表達(dá)率高于鼻咽部良性病變組織,P0.05。 2、DNA異倍體的表達(dá)與鼻咽癌臨床分期有關(guān):Ⅲ-Ⅳ期組的表達(dá)率(46.88%)明顯高于Ⅰ-Ⅱ期組(20.83%),P0.05;另外,DNA異倍體的表達(dá)也與腫瘤局部侵犯情況有關(guān):T3-T4期組的表達(dá)率(54.55%)明顯高于T1-T2期組(23.53%),P0.05;DNA異倍體的表達(dá)與患者年齡、性別、淋巴結(jié)轉(zhuǎn)移無關(guān),P0.05。CD133蛋白的表達(dá)與鼻咽癌淋巴結(jié)轉(zhuǎn)移情況有關(guān):N2-N3期組的陽性率(56.67%)顯著高于N0-N1期組(26.92%),P0.05;CD133蛋白表達(dá)與患者的年齡、性別、腫瘤局部侵犯情況及臨床分期無關(guān),P0.05。 3、DNA異倍體組放射敏感高于DNA二倍體組,P0.05。CD133蛋白陽性組放射敏感性低于CD133蛋白陰性組,P0.05。 4、DNA異倍體組放療近期療效與DNA二倍體組無顯著差異,P0.05。CD133蛋白陽性組放療近期療效與CD133陰性組相比較差,P0.05。 5、DNA異倍體與CD133蛋白表達(dá)呈正相關(guān),P0.05。 結(jié)論: 1、DNA異倍體及CD133蛋白在鼻咽癌中的陽性表達(dá)率均顯著高于鼻咽良性病變組織,提示二者可能與鼻咽癌的發(fā)生有關(guān)。 2、DNA異倍體與鼻咽癌放射敏感性有關(guān),CD133蛋白表達(dá)與放射敏感性及放療近期療效均具有相關(guān)性,提示二者在預(yù)測(cè)放射敏感性,制定個(gè)體化治療方案方面具有一定的參考價(jià)值。 3、DNA異倍體及CD133蛋白在鼻咽癌中的表達(dá)呈正相關(guān),,提示二者可能協(xié)同參與了鼻咽癌的發(fā)生發(fā)展。
[Abstract]:Objective: To detect the expression of DNA ploidy and CD133 protein in nasopharyngeal carcinoma (NPC), and to explore the relationship between them and the radiosensitivity of nasopharyngeal carcinoma (NPC) and the short-term curative effect of radiotherapy. Methods: DNA ploidy and CD133 protein expression were detected by flow cytometry and immunohistochemical staining in 56 patients with nasopharyngeal carcinoma before treatment. Another 10 specimens of nasopharyngeal benign lesions were taken as control. All patients were treated with conventional radiotherapy regimen. The radiosensitivity was evaluated according to the degree of tumor withdrawal shown by CT when the dose of radiotherapy reached 40Gy, and the short-term efficacy was evaluated according to the criteria for evaluating the efficacy of RECIST solid tumor at the end of radiotherapy. The expression of DNA ploidy and CD133 protein in 56 cases of nasopharyngeal carcinoma (NPC) was analyzed with clinical data, and the relationship between DNA ploidy and the radiosensitivity of nasopharyngeal carcinoma (NPC) and the short-term curative effect of radiotherapy was discussed.
