頻域光學相干斷層掃描對糖尿病患者黃斑部視網(wǎng)膜厚度分區(qū)測定的研究

【摘要】:目的應用頻域OCT分區(qū)測量正常人及各期糖尿病視網(wǎng)膜病變患者黃斑部視網(wǎng)膜厚度、黃斑部視網(wǎng)膜的平均厚度及黃斑部的總體積,探討2型糖尿病患者黃斑部視網(wǎng)膜厚度及體積的特點以及與正常人之間的相關性。方法 2010年1月至2011年9月在我院確診的2型糖尿病患者185例(317眼),通過常規(guī)散瞳眼底檢查、眼底照相及FFA檢查并依據(jù)2002年糖尿病視網(wǎng)膜病變國際臨床分型標準將這317眼2型糖尿病患者分為3組:糖尿病正常視網(wǎng)膜(NDR組)患者50例(89眼)、非增生性糖尿病視網(wǎng)膜病變(NPDR組)患者73例(126眼)、增生性糖尿病視網(wǎng)膜病變(PDR組)患者62例(102眼),第4組為正常組52例(98眼)。用頻域OCT對上述4組進行黃斑部的掃描,并用分析軟件進行九分區(qū)的自動分析,記錄各區(qū)視網(wǎng)膜厚度數(shù)據(jù)和黃斑區(qū)視網(wǎng)膜平均厚度及黃斑區(qū)總的體積。結果黃斑區(qū)以中心凹1mm為直徑視網(wǎng)膜厚度正常組、NDR組、NPDR組、PDR組分別為(246.37±17.50)μm、(252.13±16.24)μm、(316.28±56.99)μm、(407.90±214.89)μm。4組黃斑區(qū)平均視網(wǎng)膜厚度分別為(285.07±10.42)μm、(289.45±11.19)μm、(332.31±39.71)μm、(390.20±64.17)μm;總體積分別為(10.22±0.39)mm3、(10.12±1.03)mm3、(11.99±3.29)mm3、(13.87±5.13)mm3。與正常組相比,NPDR、PDR組黃斑部A1-A9區(qū)視網(wǎng)膜厚度、黃斑部平均視網(wǎng)膜厚度、黃斑部總體積差異均有統(tǒng)計學意義(均為P0.05);NDR組與正常組相比,A1-A9區(qū)視網(wǎng)膜厚度、黃斑部平均視網(wǎng)膜厚度、黃斑部總體積差異均無統(tǒng)計學意義(均為P0.05);NDR、NPDR、PDR3組間以上各指標兩兩比較差異均有統(tǒng)計學意義(均為P0.05)。結論黃斑區(qū)視網(wǎng)膜厚度及體積與DR程度相關。頻域OCT能定量定性地觀察糖尿病患者黃斑區(qū)的細微變化,為糖尿病患者黃斑病變早期診斷及治療提供可靠的檢測手段。
[Abstract]:Objective to measure the macular retinal thickness, the mean macular retinal thickness and the total macular volume in normal subjects and patients with diabetic retinopathy by frequency domain OCT.To investigate the characteristics of macular retinal thickness and volume in type 2 diabetes mellitus and its correlation with normal subjects.Methods 185 patients with type 2 diabetes diagnosed in our hospital from January 2010 to September 2011 were examined by routine pupil fundus examination.Fundus photography and FFA examination were used to divide 317 eyes of type 2 diabetic patients into 3 groups according to the international clinical classification standard of diabetic retinopathy in 2002.Retinopathy NPDR group (n = 73), proliferative diabetic retinopathy group (n = 62, n = 102) and normal group (n = 52, n = 98).The macular regions of the four groups were scanned by frequency-domain OCT.
