【摘要】：目的：對(duì)123例鼻阻塞患者行鼻內(nèi)鏡手術(shù)前后的鼻阻力檢測(cè)的量化數(shù)據(jù)進(jìn)行對(duì)比，探討患者手術(shù)前后鼻腔通氣功能的變化及這項(xiàng)檢查在評(píng)估手術(shù)療效中的意義。方法：應(yīng)用鼻阻力測(cè)量?jī)x對(duì)49例鼻中隔偏曲患者、36例伴鼻息肉的慢性鼻竇炎患者和38例不伴鼻息肉的慢性鼻竇炎患者術(shù)前及術(shù)后一個(gè)月的鼻阻力(nasal resistanceNR)進(jìn)行檢測(cè)，，并將檢測(cè)結(jié)果進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果：經(jīng)過(guò)鼻內(nèi)鏡手術(shù)鼻中隔矯偏曲組、伴有鼻息肉的慢性鼻竇炎組手術(shù)前后NR均有顯著差異性（P＜0.01），不伴鼻息肉慢性鼻竇炎組手術(shù)前后NR差異無(wú)統(tǒng)計(jì)學(xué)意義（P＞0.05）。結(jié)論：鼻阻力的主要產(chǎn)生的部位是鼻瓣區(qū)；變應(yīng)性因素的參與和手術(shù)前后鼻腔呼吸氣流變化也是影響鼻阻力變化的原因；正常生理狀態(tài)下和病理狀態(tài)下的鼻瓣區(qū)位置是不同的；鼻內(nèi)鏡手術(shù)并配合圍手術(shù)期綜合治療對(duì)鼻中隔偏曲，伴有鼻息肉的慢性鼻竇炎患者及不伴有鼻息肉的慢性鼻竇炎患者的鼻阻力的恢復(fù)有明顯作用；NR臨床檢測(cè)方法，簡(jiǎn)便易行，測(cè)得的數(shù)據(jù)具有良好的臨床參考價(jià)值，可對(duì)手術(shù)對(duì)改善鼻腔通氣效果進(jìn)行一定的客觀(guān)性的評(píng)估。
[Abstract]:Objective: to compare the quantitative data of nasal resistance in 123 patients with nasal obstruction before and after endoscopic surgery.Methods: nasal resistance was measured in 49 patients with deviation of nasal septum and 38 patients with chronic sinusitis without nasal polyps before and one month after operation.The results were analyzed statistically.Results: there was significant difference in NR before and after operation in patients with chronic sinusitis with nasal polyps after nasal endoscopic surgery (P < 0.01), but there was no significant difference in NR before and after operation in patients with chronic sinusitis without nasal polyps (P > 0.05).Conclusion: the main position of nasal resistance is the nasal flap area, the involvement of allergic factors and the changes of nasal respiratory and airflow before and after operation are also the causes of nasal resistance change, the position of nasal flap region is different in normal physiological state and pathological state.The recovery of nasal resistance in patients with nasal septum deviation, nasal polyps and chronic sinusitis without nasal polyps, combined with perioperative comprehensive treatment, has obvious effect on the recovery of nasal resistance of patients with nasal septum deviation, nasal polyps and chronic sinusitis without nasal polyps.The measured data have good clinical reference value and can be used to evaluate the effect of surgery on improving nasal ventilation.
