【摘要】：目的比較耳內(nèi)鏡下耳屏軟骨-軟骨膜鼓膜修補術和傳統(tǒng)顯微鏡下顳肌筋膜鼓膜修補術的療效。方法回顧2011年1月至2014年12月于我科接受鼓膜修補術的患者460例,按手術方式分為2組,一組耳內(nèi)鏡下耳屏軟骨-軟骨膜鼓膜修補術(耳內(nèi)鏡組),共242例(242耳),另一組顯微鏡下顳肌筋膜鼓膜修補術(顯微鏡組),共218例(218耳)。比較兩組手術時間、術中出血量,術后1個月、3-6個月、1-2年鼓膜愈合率的情況。結果耳內(nèi)鏡組平均手術時間(43.5±14.2)min,平均術中出血量(5.2±1.8)ml,而顯微鏡組平均手術時間(84.0±13.5)min,平均術中出血量為(15.0±4.1)ml,差異均有統(tǒng)計學意義(t=-6.55,P0.01;t=-12.14,P0.01);術后1個月耳內(nèi)鏡組鼓膜愈合率為94.21%,顯微鏡組為91.74%;術后3-6個月、術后1-2年隨訪,長期鼓膜愈合率耳內(nèi)鏡組和顯微鏡組鼓膜愈合率分別為92.15%和89.45%,差異均無統(tǒng)計學意義(P0.05)。結論耳內(nèi)鏡下耳屏軟骨-軟骨膜修補鼓膜穿孔可以獲得較好的愈合率,是一種有效的手術方法,值得一定范圍內(nèi)臨床推廣。
