【摘要】：評價角膜繃帶鏡治療角膜穿孔及近穿孔的臨床效果。收集2014年6月~2015年6月期間因角膜穿孔或近穿孔角膜在浙江大學醫(yī)學院附屬第二醫(yī)院眼科中心接受角膜繃帶鏡治療,并在配戴前后評估患者主觀癥狀,使用裂隙燈檢查,眼壓,眼前節(jié)照相,角膜熒光素染色,最佳矯正視力等指標對患者眼表進行綜合評價。并記錄整個隨訪過程中戴角膜繃帶鏡可能引起的并發(fā)癥。所有患者在停戴繃帶鏡后均隨訪3月以上。研究期間共有符合要求的病例19例(19眼),這些患者中包括暴露性角膜炎引起角膜潰瘍穿孔4例,角膜穿孔傷4例,反復病毒性角膜炎導致的神經營養(yǎng)性角膜潰瘍穿孔3例,移植物抗宿主反應病導致3例角膜穿孔,1例翼狀胬肉術后角膜潰瘍穿孔,3例白內障術后角膜潰瘍穿孔,1例化學傷引起。19例患者治療結果:10例近穿孔的角膜病變患者治療后角膜潰瘍面積愈合,穿孔創(chuàng)面修復,沒有發(fā)生角膜穿孔;8例全層角膜穿孔的患者治療后角膜創(chuàng)口閉合,前房深度恢復,眼表形態(tài)結構得到有效恢復;1例全層角膜穿孔角膜繃帶鏡配戴不穩(wěn)脫落。在角膜繃帶鏡佩戴期間沒有發(fā)現(xiàn)任何繼發(fā)感染及其他并發(fā)癥。角膜繃帶鏡是治療角膜穿孔或近穿孔的有效手段,是一種安全有效的輔助治療方法。
[Abstract]:To evaluate the clinical effect of corneal bandage in the treatment of corneal perforation and near-perforation.From June 2014 to June 2015, patients with corneal perforation or near perforation were treated with corneal bandage in the Eye Center of the second affiliated Hospital of Zhejiang University Medical College. The subjective symptoms of the patients were evaluated before and after wearing, and the intraocular pressure was examined with slit lamp.Anterior segment photography, corneal fluorescein staining and best corrected visual acuity (BCVA) were used to evaluate the ocular surface of the patients.The possible complications of corneal bandage during the follow-up were recorded.All patients were followed up for more than 3 months after bandage.During the study period, there were 19 cases of corneal ulcer perforation caused by exposure keratitis, 4 cases of corneal perforation injury and 3 cases of neurotrophic corneal ulcer perforation caused by repeated viral keratitis.Graft-versus-host response disease results in 3 cases of corneal perforation, 1 case of corneal ulcer perforation after pterygium, 3 cases of corneal ulcer perforation after cataract surgery, 1 case of chemical injury, and 1 case of chemical injury.The area of corneal ulcer healed after treatment.After treatment, the corneal wound was closed, the depth of anterior chamber was restored, and the morphology and structure of the ocular surface were recovered effectively.No secondary infections or other complications were found during the corneal bandage wear.Corneal bandage is an effective method for the treatment of corneal perforation or near-perforation, and is a safe and effective adjuvant treatment.
