【摘要】：目的探討阻塞性睡眠呼吸暫停低通氣綜合征(obstructive sleep apnea hypopnea syndrome,OSAHS)與胃反流性疾病(gastroesophageal reflux disease)的關(guān)系。方法 2013年2月至2014年6月期間就診于新疆維吾爾自治區(qū)人民醫(yī)院的OSAHS患者30例,采用多導(dǎo)睡眠監(jiān)測(cè)、24 h多通道腔阻抗-p H監(jiān)測(cè)和高分辨率胃腸動(dòng)力學(xué)檢查系統(tǒng)進(jìn)行監(jiān)測(cè),觀察食管上、下括約肌、食管動(dòng)力學(xué)特點(diǎn)及反流事件相關(guān)指標(biāo),并與10例健康成人對(duì)照組比較,比較組間各測(cè)量指標(biāo)差異。結(jié)果OSAHS組較對(duì)照組食管上括約肌松弛持續(xù)時(shí)間、食管上括約肌松弛恢復(fù)時(shí)間、食管下括約肌長(zhǎng)度、遠(yuǎn)端收縮積分平均值均減小,收縮前沿速度增加,差異均具有統(tǒng)計(jì)學(xué)意義(t=-2.061、-2.044、-2.525、-2.076、2.522,P均0.05)。OSAHS組與對(duì)照組中De Meester評(píng)分中位數(shù)(M[P25;P75])分別為11.3[5.1;37.8]、3.8[2.8;11.1]分。食管近端反流總次數(shù)分別為41.5[23.8;65.3]、24.5[16.3;27.8]次,酸反流次數(shù)分別為20.0[10.0;32.0]、10.5[7.8;14.3]次。OSAHS組食管近端反流總次數(shù)、酸反流次數(shù)及De Meester評(píng)分均較對(duì)照組增加,差異均有統(tǒng)計(jì)學(xué)意義(z=-2.438、-2.361、-2.140,P均0.05)。結(jié)論 OSAHS部分患者存在食管上、下括約肌結(jié)構(gòu)和功能障礙,存在胃食管反流現(xiàn)象,食管近端反流以酸性物質(zhì)為主。
[Abstract]:Objective to investigate the relationship between obstructive sleep apnea hypopnea syndrome (OSAHS) and gastroesophageal reflux disease (GRS) in patients with obstructive sleep apnea hypopnea syndrome (OSAHS).Methods from February 2013 to June 2014, 30 patients with OSAHS were treated in Xinjiang Uygur Autonomous region people's Hospital. The patients were monitored by multi-channel cavity impedance (-pH) monitoring and high-resolution gastrointestinal dynamic examination system for 24 h with polysomnography.The upper esophageal sphincter, lower sphincter, esophageal dynamics and reflux events were observed and compared with 10 healthy adults.Results the relaxation duration of upper esophageal sphincter, the recovery time of upper esophageal sphincter relaxation, the length of lower esophageal sphincter, the mean value of distal contraction integral in OSAHS group were decreased, and the velocity of contraction front increased.The difference was statistically significant (P < 0.05).
